Abstract

Diabetes is one of the main reasons for pathogenicity, death and increase in the cost of health services all over the world, and in case it occurs simultaneously with depression, the extent and severity of the symptoms will increase, as depression is one of the most prevalent psychological disorders in diabetic people which can result in more undesirable changes in the prognosis of the disease. Therefore, this study was conducted to determine the simultaneous incidence of diabetes and depression and some related issues. This

The incidence of depression together with diabetes causes a decrease in metabolic control, medication adherence, decreased quality of life, failure to follow appropriate diets, increase in the costs of health care, incapability, death, and increase in the use of insulin and diabetic symptoms. These illnesses underlie numerous illnesses such as dementia disorders and affect the entire process of treatment.

Diabetes, like other medical disorders, has a considerable interaction with depression and regarding that the incidence of depression in diabetic people is high, it is necessary to screen depression and to provide remedies such as education in order to improve the coping ability in these patients.

Abstract

Background and objective:

Little studies using a limited number of questionnaires cannot reflect the depth and complication of diabetic patients about self-care. However, using the qualitative research method, it is possible to find out the depth of patients’ experiences of this illness. The purpose of this study was to find individual and environmental obstacles which affect self-care on these patients and to investigate patients’ viewpoint of this disease.

Materials and Methods:

In this qualitative study, group interview and case interview have been used. This study was carried out in two phases and at the end of the interview patients were provided with necessary teaching about the principals of self-care.

Results:

In general, 5 categories of obstacles to self-care in type 2 diabetes were identified which include: physical obstacles, economical obstacles, social obstacles, educational obstacles, and psychic obstacles. Social, educational and psychic obstacles are in latter classes.

Conclusion:

The results of this study showed that a high percentage of patients with type 2 diabetes face serious obstacles in the way of self-care and the most numerous and important obstacles in their opinion are physical and economical obstacles. Considering that developing countries face a shortage of resources, dealing with physical obstacles can bring satisfaction to patients and on the other hand, dealing with these obstacles requires less cooperation from exterior organizations.

Abstract

Background & Objective: Dyslipidemia increases the risk of developing cardiovascular diseases in diabetic patients. Lipoprotein a, as a cholesterol carrier and competitive inhibitor of plasminogen in the blood coagulation system is considered an atherogenic factor which increases the risk of developing atherosclerosis in diabetic individuals. Based on previous studies, Barberry is thought to lower cholesterol and triglyceride levels. Since increased cholesterol level, especially Lipoprotein a, increases the risk of cardiovascular diseases in people with diabetes, the aim of this study was to evaluate serum levels of Lipoprotein (a) in diabetic rats treated with aqueous extract of Barberry. Method: In this experimental study, diabetes was induced in 15 mice by intraperitoneal injection of streptozotocin (STZ) and then they were treated orally for 14 days with liquid extract of the said plant in doses of 25 and 100 mg/kg. At the end of the treatment period, mice were kept fasted for 12 hours and then by heart phlebotomy, serum levels of Lipoprotein a, serum levels of glucose, triglycerides, total cholesterol, HDL and LDL were measured in normal and diabetic groups. The results were then analyzed using SPSS version 16, ANOVA and Tukey›s test. Results: Data analysis showed that the consumption of both oral doses of Berberis vulgaris (Barberry) in diabetic rats causes a significant reduction in serum levels of triglyceride, serum glucose and VLDL(P=0.01). The extract at both used doses also significantly reduced serum levels of lipoprotein (a) in treated diabetic rats (P=0.012). Conclusion: Oral administration of the aqueous extract of Berberis vulgaris (Barberry) is effective for reducing triglyceride and glucose levels in diabetic mice. Since Barberry lowers Lipoprotein (a) levels, it can be considered as an anti-atherogenic factor.

Abstract

Background and objective: Type 2 diabetes (T2D) is a metabolic disease that leads to increased levels of lipids, therefore the present study aimed to investigate the effect of eight weeks of aerobic training(AT) with silymarin (S)consumption on lipid profile in men with type 2 diabetes.
Material and Methods: 60 middle-aged male volunteers were randomly divided into 4 groups, including: (1) control(C) + placebo, (2) AT + placebo, (3) S consumption and (4) AT+S consumption. The AT groups performed for eight weeks, three sessions per week and 20-45 minutes per session at an intensity of 60 to 85% of the reserve heart rate, and the S groups of consumed 140 mg / kg of S daily (in two meals).  Serum levels of Low Density Lipoprotein (LDL), High Density Lipoprotein (HDL), Cholesterol (CHO), and Triglyceride (TG) were measured before and after the test. Data were analyzed using one-way ANOVA and Bonferroni’s test, and P value less than 0.05 was considered significant.
Results: AT reduced serum levels of LDL, CHO, TG and increased HDL in men with T2D(P ≥ 0.05). Consumption of S reduced LDL, CHO, TG and increased HDL in men with T2D(P ≥ 0.05). AT and S consumption reduced LDL, CHO, TG and increased HDL; also, decrease in LDL, CHO, TG and increase in HDL in the AT and S consumption group was more favorable than the effect of S alone (P ≥ 0.05).
Conclusion: It seems that AT and S consumption simultaneously have interactive effects on reducing LDL, CHO, TG and increasing HDL in men with T2D.

Abstract

Background and objective: Currently, due to an increase in the number of individuals with immune deficiency, long-term chemotherapy, and underlying diseases, an appropriate situation has been provided for the development of opportunistic infections, including fungal infections. This study was conducted with the aim of evaluating clinical symptoms, laboratory findings, and the treatment outcome of mucormycosis in diabetic individuals.
Material and Methods: In this cross-sectional retrospective study, all recorded cases of mucormycosis in the health centers of Gorgan city, northeast of Iran, in diabetic individuals were extracted during 15 years from 2002 to 2016. All information was extracted from patient-related records and then was analyzed. In the period from 2002 to 2016, 12 diabetic individuals with mucormycosis were referred to health centers.
Results: There were statistically significant differences in the categories of sex, residence, education, taking drug, addiction, diabetes status, methods of diagnosis, involved area with mucormycosis, treatment type of mucormycosis, and treatment outcome of mucormycosis. Furthermore, no statistically significant difference was observed in the categories of age, underlying diseases (other than diabetes), hospitalization, and diagnostic time.
Conclusion: Due to the rareness or lack of an appropriate diagnostic method, and in addition, due to lack of an appropriate treatment, attention should be paid to invasive mucormycosis in individuals with immunodeficiency.

Abstract

Background and Objectives: Currently, diabetes is one of the leading causes of death in the world. According to several factors diagnosis of this disease is complex and prone to human error. This study aimed to analyze the risk of having diabetes based on laboratory information, life style and, family history with the help of machine learning algorithms. When the model is trained properly, people can examine their risk of having diabetes.
Material and Methods: To classify patients, by using Python, eight different machine learning algorithms (Logistic Regression, Nearest Neighbor, Decision Tree, Random Forest, Support Vector Machine, Naive Bayesian, Neural Network and Gradient Boosting) were analysed. were evaluated by accuracy, sensitivity, specificity and ROC curve parameters.
ResultsThe model based on the gradient boosting algorithm showed the best performance with a prediction accuracy of %95.50.
ConclusionIn the future, this model can be used for diagnosis diabete. The basis of this study is to do more research and develop models such as other learning machine algorithms.

Abstract

Background and objective: The effect of aerobic exercise (AE) on cardiovascular (CVD) related risk factors are still debatable. Therefore this randomized controlled clinical trial (RCT) was performed with unique and specific AE protocol to investigate the effect of eight-week AE only on female subjects with type 2 diabetes.
Material And Method: A controlled RCT was performed on 30 women with type 2 diabetes aged 30 - 50. They were randomly divided into two groups, intervention and control by block randomization method. The intervention group received an incremental AE for eight weeks, three sessions per week, max heart rate (55-75%) and rating of perceived exertion (RPE) 12-13 for 150 minutes per week. Blood samples were taken before and after each intervention for both groups to be evaluated for fasting blood sugar (FBS), hemoglobin A1c (HbA1c), lipid profile [triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C)].
Result: Eight-week AE intervention led to a significant decrease in HbA1c, FBS levels (p = 0.025, p=0.003) in experimental group compared to control however no significant differences was observed in terms of lipid profile (total cholesterol (TC), triglyceride (TG), LDL-C and HDL-C) between control and experimental group (p> 0.05). The results of paired t-test showed that FBS, TC and HbA1c levels were significantly reduced within experimental group after intervention compared to pre-test (p = 0.038, p = 0.05, p = 0.002, respectively). There was no significant difference between TG, LDL-C, and HDL-C levels within experimental group between pre and post-test (p>0.05).
Conclusion: It can be concluded that AE has been effectively reduce FBS, HbA1c and TC in women type 2 diabetes.

Abstract

Background and Objective: Although the efficacy of selected training and antioxidant herbs in the treatment of diabetes mellitus has been determined, the interactive effect of selected training and aqueous extract of barberry on insulin resistance and risk factors of type2 diabetes mellitus is not well understood. The aim of the present study was to investigate the effect of aqueous extract of barberry and selected training on some blood factors in men with type2 diabetes.
Material and Methods: In this study, 48 middle-aged men an age range of 40-50 years with type2 diabetes participated in the study. The participants were randomly divided into four groups of 12 subjects, including(1)control, (2)aqueous extract of barberry consumption, (3)aerobic training, and (4)aerobic training+ barberry consumption, based on insulin resistance index. The participants in the aqueous extract of barberry consumption group received200 ml of barberry juice daily for8 weeks and the training and training+ barberry consumption groups performed the training protocol designed in the study for 8 weeks, while the control group did not receive any intervention.
Results: After 8 weeks of exercise training and consumption of aqueous extract of barberry, insulin resistance index, fasting insulin level, fasting blood glucose level, triglyceride level, low density lipoprotein (LDL)and total cholesterol were significantly decreased compared to the control group and high density lipoprotein (HDL)had a significant increase; on the other hand, regarding the intergroup changes, only the levels of LDL and CHO were significant compared to the aqueous extract of barberry consumption and aerobic training.
Conclusion: In general, aqueous extract of barberry consumption and regular exercise training seems to be effective in improving insulin resistance index and blood lipids levels in type2 diabetic patients.

Abstract

Background and Objective: Diabetes is one of the health problems in all societies. The aim of the present study was to investigate the effect of a period of training at different temperatures on the gene expression of GLUT_4 and insulin receptor in the brown adipose tissue of diabetic rats.
Material and Methods: In this experimental study, 21 diabetic rats were randomly divided into 3 groups of 7 animals, including: (1) control (C), (2) swimming training at 5° C (S5ºC), and (3) swimming training at 36° C (S36ºC). Water swimming training was performed at 5±2° C and 36±2° C for six weeks, 5 sessions per week and 2-4 minutes per session. Data were analyzed using one-way analysis of variance and Tukey’s post hoc test at the significance level of p≤0.05.
Results: The gene expression of GLUT4 and insulin receptor in the S5º C and S36º C groups was significantly (P = 0.0001) higher than the control group. Also, the gene expression of GLUT4 in the S36º C group was higher than the S5º C group (p = 0.001), and the expression of insulin receptor in the S5º C group was significantly (p = 0.001) higher than the S36º C group.
Conclusion: Swimming training at 5º C and 36º C significantly increased the gene expression of GLUT4 and insulin receptor in the brown adipose tissue of diabetic rats.

Abstract

Background and Objectives: As a health emergency of the 21st century, Diabetes is associated with an increased risk of cardiovascular diseases, stroke, diabetic nephropathy, neuropathy, retinopathy, pregnancy complications, psychological problems, lower quality of life experience, as well as a high economic burden and high risk of premature death. This study aimed to develop psychological interventions headings for patients referring to healthcare service centers based on local needs assessment through content analysis method of experts' opinion to provide services in the primary healthcare system.
Material and Methods: This study was conducted on 19 mental health specialists, professors, and the health team providing services in healthcare service centers (family doctor and mental health expert) in 2018-19. The data obtained from the semi-structured interviews were analyzed using the directed content analysis technique.
Results: The essential finding themes related to the closed headings of psychological interventions in diabetes for improving disease management included "collaborative care," "psychological assessment," the importance of "diabetes coexistence with psychiatric disorders," "creating health behaviors," "self-management training," and "intervention" In crisis" for the management of type 2 diabetes.
Conclusion: This qualitative study was conducted with the participation of mental health experts. Therefore, the mentioned themes and categories can be used in developing the headings of psychological intervention programs for type 2 diabetic patients, focusing on the needs of Iranian patients and the structure of the health system.

Abstract

Background: Type 2 diabetes is a metabolic disease associated with a partial or absolute deficiency of insulin, hyperlipidemia, and impaired metabolism of carbohydrates, fats, and proteins.
The present study aimed to compare the effects of different types of exercises on lipid profiles in menopausal women with type 2 diabetes.
Methods: Fifty-six sedentary obese menopausal women with type 2 diabetes were randomly divided into 4 groups: aerobic exercise, resistance exercise, combined exercise, and a control group. Each group, except the control group, participated in their respective exercises for 8 weeks. Blood samples were collected from the subjects before and 48 hours after the training period. Data were analyzed using the analysis of variance (ANOVA) and paired sample t tests, with a significance level set at P≤0.05. All calculations were performed using SPSS v. 20.
Results: Triglyceride levels significantly decreased in the aerobic exercise (P = 0.017) and combined exercise (P = 0.0001) groups. Low-density lipoprotein levels significantly decreased in aerobic exercise (P = 0.034), resistance exercise (P = 0.003), and combined exercise groups (P = 0.009). Cholesterol levels significantly decreased in the aerobic exercise (P = 0.0001), resistance exercise (P = 0.030), and combined exercise (P = 0.01) groups. Additionally, high-density lipoprotein levels significantly increased in the aerobic exercise (P = 0.041) and combined exercise (P = 0.002) groups.
Conclusion: It appears that combined exercises, rather than resistance or endurance exercises alone, are a more appropriate intervention for improving lipid profiles in menopausal women with type 2 diabetes.

Abstract

Background: The use of complementary medicine in healthcare is increasing rapidly. Therefore, the aim of the current study was to investigate the effect of eight weeks of high-intensity interval training and moderate-intensity continuous training with quercetin supplementation on the gene expression of FOXO1 and ATG5 in the liver of diabetic obese rats.
Methods: In this experimental study, 42 male Wistar rats were considered research samples at eight weeks. Rats were induced with diabetes after eight weeks of a high-fat diet and familiarization with a laboratory environment and treadmill. Rats were divided into seven groups, and six rats were placed in each group (n=6): healthy control group, diabetes control group, diabetic quercetin group, high-intensity interval training with diabetes group, moderate-intensity continuous training with diabetes group, diabetic high-intensity interval training with quercetin group, and diabetic moderate-intensity continuous training with quercetin group. For one training group, eight weeks of high-intensity interval training, and for the other group, eight weeks of moderate-intensity continuous training on the treadmill were performed. Seventy-two hours after the last training session, liver tissues were isolated to check the gene expression of FOXO1 and ATG5. One-way analysis of variance test was used to check the difference between groups by SPSS version 26 software.
Results: Findings showed that by inducing type 2 diabetes, gene expression of FOXO1 increased (3.14 unit) (P<0.001) and ATG5 gene expression decreased (0.71 unit) (P<0.001). After eight weeks of training investigation, gene expression of FOXO1 and ATG5 decreased (P<0.001) and increased (P<0.001), respectively, in all training groups compared to the diabetic control group. There was no significant difference between the four training groups (P>0/05).
Conclusion: Both exercises with and without quercetin had a modulating effect on the gene expression of indicators related to the process of autophagy and blood glucose levels in the liver of diabetic obese rats.
