Abstract

Abstract Background and Objective: Stenotrphomonas maltophilia is an opportunistic nosocomial pathogen with high mortality in immunocompromised cases. The aim of this study was to isolate and identify Stenotrphomonas maltophilia in the hospitals’ environment and wards. Material and Methods: In this cross-sectional study, a total of 1108 samples were collected from environment of two hospitals during 12 months. Identification of isolates was performed using biochemical, phenotypic (intrinsic resistance to carbapenems) and molecular methods (amplification of 23S rRNA gene). Results: Of the studied samples, 186 (16.78%) nonfermentative gram negative bacilli (NFGNB) were identified. Amongst NFGNB, 18 (1.62%) isolates were identified as S. maltophiliaby using biochemical tests. Of 18 biochemically identified isolates, 15 (83.3%) were confirmed via PCR. Sinks (40%) and men surgery ward ( 33.3 %( were the most contaminated sites and wards of hospitals, respectively. Conclusion: S. maltophilia is repeatedly isolated from sink which shows that the moist hospital environments need to be considered as a source for dissemination of bacteria. Abstract

Abstract

Background and Objective: Oral candidiasis is a common opportunistic infection in diabetic patients. Candida albicans is colonized on different oral surfaces such as tongue, palate, dental caries and plaques.  This study aimed to determine the prevalence of   Candida albicans in the mouth of   diabetic patient.

Material and Methods: This cross - sectional study was conducted on 120 diabetic patients. After completing the questionnaire, the   saliva samples were obtained and cultured   on sabouraud agar medium containing 0.005% chloramphenicol. For final diagnosis, Germ tube test and Sugar absorption test (with API 120 kit) were used.

Results:  Of 120 patients, 61 (50.8%) were positive for Candida.  Candida infection rate was relatively higher in older patients and in women.  The rate was significant in the patients using dentures (P< 0.001) and   the ones keeping on smoking (P <0.038).  The infection in the patients with type II diabetes was higher than the cases with Type I.

Conclusion: Given the high prevalence of Candidiasis and Candida colonization in the mouth of diabetic patients, it is a necessity to identify and treat these infections in order to prevent further complications.

Abstract

Abstract

     Background and Objective: Diabetes mellitus is the most common risk factor for coronary artery disease (CAD). Cholesteryl ester transfer protein (CETP) TaqIB polymorphism is associated with changes in lipid profile and may be a risk factor for CAD in patients with diabetes. This study aimed to evaluate the association of CETP TaqIB polymorphism with CAD in patients with type 2 diabetes.

     Methods: In this case-control study, 292 diabetic patients were divided into two groups based on angiography reports (150 participants with normal angiogram as the control group and 142 participants with more than 50% stenosis of at least one coronary artery as the case group). The CETP TaqIB genotypes were determined by PCR-RFLP analysis. Fasting blood glucose was measured using glucose oxidase and lipid profile (triglycerides, total cholesterol, high density lipoprotein-cholesterol and low density lipoprotein-cholesterol) by an enzymatic method.

       Results: There was no significant difference in the frequency of genotypes and alleles between the case group and controls (the control group: B1B1, 17.3%; B1B2, 63.3%; and B2B2, 19.3%; the case group: B1B1, 18.3%; B1B2, 64.1%; and B2B2, 17.6%) (P=0.92). In the control group, heterozygous participants (genotype B1B2) had higher levels of cholesterol compared with other genotypes (B1B1 and B2B2). Also, the patients with genotype B1B2 had significantly higher weight (P=0.013).

       Conclusion: There is no significant correlation between CETP TaqIB polymorphism and the increased risk of coronary artery disease in patients with type 2 diabetes.

Abstract

ABSTRACT

         Background and Objective: Vitamins C and E are  the two main antioxidants involved in prevention of type 2 diabetes, by reducing oxidative stress. The aim of this study was to evaluate the effects of vitamins C and E supplementation independently, on serum levels of insulin, high sensitivity C-reactive protein (hs-CRP) and resistin in people with type 2 diabetes.

        Methods: In this study, 38 patients with type 2 diabetes (17 men and 21 women) received 1,000 mg/day vitamin C, and 40 patients with type 2 diabetes (21 men and 19 women) received 400 IU/day vitamin E orally. Fasting blood glucose and lipid profile were measured using enzymatic method. Hs-CRP was measured by immunoturbidimetric method, and serum insulin and resistin levels were measured by ELISA.

         Results: total cholesterol, triglycerides, hs-CRP, insulin and resistin significantly were reduced after vitamin C supplementation (P<0.001). Moreover, the level of total cholesterol (P =0.018), low-density lipoprotein, triglycerides and hs-CRP significantly changed after vitamin E supplementation (P=0.001).

          Conclusion: The daily intake of 1,000 mg of vitamin C and 400 IU of vitamin E may be useful in reducing diabetic complications by decreasing serum levels of hs-CRP and lipid profile in people with type 2 diabetes.

Abstract

ABSTRACT
          Background and Objectives: Determining the genetic relationship between S. aureus isolates is important for epidemiological surveillance and control of infections caused by this bacterium. The present study was conducted to determine polymorphisms of coagulase gene (coa) among S. aureus isolates from pastry and cheese samples using restriction fragment length polymorphism (RFLP) analysis.
         Methods: Overall, 65 S. aureus isolated from pastry (n=45) and cheese (n=20) samples were examined for the coa gene by polymerase chain reaction (PCR). PCR products were digested with AluI enzyme and the products were assessed using gel electrophoresis.
          Results: Except for two isolates, all isolates were positive in coa-PCR and produced four different PCR products, with molecular sizes ranging from 570 to 970 bp. Overall; five distinct RFLP patterns were detected (I-V). Although pattern types I and III were present in isolates from both samples, types I and IV were mainly present in isolates from cheese and pastry samples, respectively.
        Conclusion: PCR-RFLP analysis of the coa gene indicates that S. aureus isolates from pastry and cheese samples may be originated from different sources. However, as one pattern type was predominant in each group, it can be concluded that majority of the isolates may have the same origin.

Abstract

ABSTRACT
           Background and Objectives: Halophilic bacteria can grow and survive in environments with a wide range a wide range of salinities. In this study, we aimed to isolate halophilic bacteria from the Meyghan Lake in Arak (Iran) and evaluate their enzymatic activity.
           Methods: Samples were taken from four different areas of the lake. Halophilic bacteria were isolated by culture in moderate halophilic medium, sea water nutrient agar and nutrient agar containing different salt concentrations. Purification was done via consecutive culture methods, and production of extracellular hydrolytic enzymes including amylase, protease, lecithinase, DNase and lipase was evaluated.
           Results: Among 74 bacteria isolated from the lake water samples, 24 produced amylase, 27 produced lipase (Tween 40 and 80), 68 produced protease, three produced DNase and 61 produced lecithinase.
           Conclusion: In this study, we isolated halophilic bacteria with enzymatic activity and potential industrial applications.

Abstract

ABSTRACT
          Background and objectives: Nosocomial infections caused by antibiotic resistant bacteria is a life threatening health challenge. This study aimed to determine the frequency of antibiotic resistance genes in clinical isolates from hospitals of Zahedan, southeast of Iran.
           Methods: Overall, 818 isolates were collected from different hospital wards. The isolates were identified using conventional microbiological and biochemical tests. Antibiotic susceptibility pattern was assessed by agar disc diffusion method and determination of minimum inhibitory concentration of a number of antibiotics. Multiplex PCR was performed using specific primers for the detection of resistance genes.
           Results: The most common species were Staphylococcus aureus (25%), Klebsiella pneumoniae (22%) and Pseudomonas aeruginosa (14%). The rate of methicillin resistance among S. aureus, S. epidermidis and S. saprophyticus was 60%, 43% and 24%, respectively. In addition, 28.5% of enterococci isolates were vancomycin resistant. Among gram-negative bacteria, 45% of A. baumannii and 24% of P. aeruginosa were identified as ESBL. A high level of resistance to ampicillin (96%), cefotaxime (89%), gentamicin (89%) and sulfamethoxazole-trimethoprime (60%) was observed in K. pneumoniae.
           Conclusion: Our results highlight the urgent need for an eradication program and a surveillance plan for preventing increased emergence of antibiotic resistant bacteria in the study area.

Abstract

Background and objectives: Dried Black Curd (DBC), also known as Gharahghorut (Persian), is a dairy product produced from curd of yogurt or doogh. The aim of this study was to evaluate microbial contamination of industrially and traditionally produced DBC in Iran.
 
Methods: Four DBC brands holding certification of the Institute of Standards and Industrial Research of Iran (ISIRI), Iran Ministry of Health and Medical Education (IMHME) and ISO were purchased from a market in Tehran, Iran. Microbial monitoring for presence of coliform, Escherichia coli, coagulase-positive Staphylococcus aureus, mold and yeast was performed using different basic, enriched, selective and differential media including peptone water agar, MacCankey agar, blood agar and brain heart infusion agar, etc. according to the ISIRI standards (No.13299).
 
Results: None of the tested samples had microbial contamination. However, in one traditional DBC sample, the population of mold and yeast was higher than the acceptable level defined by the ISIRI standard.
 
Conclusion: We found no contamination with coagulase-positive S. aureus, E. coli and coliforms in four brands of traditionally and industrially produced DBC in Iran. This indicates that the hygienic practices designated by the IMHME are well-implemented in the industrial sector. However, the mold and yeast contamination in traditionally produced DBC should be prevented by applying hygienic practices during the process of manufacturing and distribution.

Abstract

Background and objectives: Gouda Cheese is regarded as a high quality and one of the most popular cheeses in the world. The defining characteristics of Gouda cheese are its yellow color, great aroma and taste of caramel sweetness. The cheese should be well chilled before waxing, to get better seal. The seal is very important to prevent contamination with molds and putrefactive bacteria. Cheese wax is made from paraffin with additional microcrystalline to make pliable for better seal. The aim of this study was to investigate antibacterial effects of Gouda cheese wax.
Methods: Gouda cheese wax samples were collected from four different manufacturers in Iran. The total count of coliforms, Escherichia coli, Salmonella, coagulase-positive Staphylococcus and mold and yeast on the samples was determined. The antimicrobial activities of Gouda cheese wax against E. coli, S. aureus, Saccharomyces cerevisiae, Aspergillus brazilissis and Salmonella enterica were investigated by determining minimum bactericidal concentration and minimum inhibitory concentration.
Results: The results indicated that all Gouda cheese wax samples were prepared in accordance with the national standards. In addition, the examined wax samples had no antimicrobial properties against the tested microorganisms.
Conclusions: The wax used in production of Gouda cheese in Iran has no antimicrobial properties.

Abstract

Increasing antibiotic resistance is a global health problem. In recent years, due to the indiscriminate use of antibacterial compounds, many bacterial pathogens, including staphylococci, members of the Enterobacteriaceae family including Klebsiella pneumoniae and bacteria such as Pseudomonas aeruginosa and Acinetobacter baumannii have become multi-drug resistant. Consequently, it is important to explore alternative approaches for eliminating resistant strains. Bacteria synthesize low-weight molecules called siderophores to chelate iron from the environment as a vital element for their growth and survival. One way to deal with resistant bacterial strains is to utilize siderophore-mediated iron uptake pathways as entrance routes for drug delivery. Therefore, the production of drugs with Trojan horse strategy in the form of conjugated siderophore-antibiotic complexes has recently received much attention for dealing with resistant isolates. In this review, we discuss the efficacy of siderophore-antibiotic conjugates as a Trojan horse strategy for eliminating drug-resistant pathogens.
