Abstract
Abstract Background and objectives: Food handlers could be the main sources of intestinal parasite transmission in case of not observing the hygienic rules. Contamination can be decreased by screening food handlers through physical exam and laboratory tests. The aim of this study was determining the prevalence of intestinal parasites in 2010. Material and Methods: This cross-sectional research was carried out on 500 randomly selected individuals engaged in different food related careers. After filling out the questionnaire sheets, two specimens of feces were collected from each person and tested by brine 30% (floatation) and direct methods. Result: The results show that the prevalence of intestinal parasites is 6%. The highest prevalence is relateted to Giardia lamblia ( 17 4.3%) and the lowest to Hymenolepis nana ( 3 0.6%). in the age group of 60-51 years (11.8%) and individuals who just able to read and write (7.4%) The highest percentage is observed. The Most contamination is reported in butchery staff (25%) and the lowest in people worked in butler's pantry, without parasitic infections. Conclusion: The prevalence of intestinal parasitic infections are high relatively, especially pathogenic protozoan therefore, it is important be careful about health status of these individuals and their role in the spread of pollution. Background and Objective: In many countries, especially in developing countries, diarrheal diseases is one of the most important causes of mortality. In these countries, due to availability transfer conditions of parasitic infections, (pathogenic and non-pathogenic parasite), have a high prevalence. This study aimed was determining the prevalence of Parasites causing diarrhea in Gorgan .
Material and Methods: This cross-sectional study was performed on 1086 Patients with acute diarrhea. The test was performing by the wetmount and formalin-ether concentration method according to WHO protocol.
Results: From 1086 patients in study, 197 (18.1%) were infected by intestinal parasites, that 141 (13%) were infected with Protozoan and 79 (7.27%) were infected with intestinal worms. The prevalence of wet mount method was, Entamoebahistolytica in 69 (6.4%), Giardia lamblia in 27 (2.5%), Entamoeba coli in 15 (1.4%), Chilomastixmesnili in 8 (0.7%), and with formalin-ether method was Hymenolepis nana in 26 (2.4%), Trichostrongylus in 16 (1.5%), Enterobiusvermicularis in 10 (0.9%).
Conclusion: The high prevalence of Entamoeba histolytica, is indicating protozoan importance as a cause of acute diarrhea.

Background and Objective: C - reactive protein (CRP) is an acute phase protein produced in liver. It is less than 5 mg per deciliter in the serum and body fluids of normal individuals, but it is increased suddenly within a few hours following inflammatory reaction. In bacterial and viral infections, active rheumatic fever, acute myocardial infarction and rheumatoid arthritis are also increased. The aim of this study was to investigate CRP level by Qualitative and Quantitative methods.
Material and Methods: The CRP of 200 patients was investigated by quantitative and qualitative methods. Qualitative CRP testing was conducted three times by different people, using two kit of bionic and Omega, and then the mean of the results was reported. For quantitative CRP testing, Immunoturbidimetry was used.
Results: In qualitative CRP test by Bionic kit: 180 (90%) were negative, 6 (3%) weakly positive, 9 (4.5%) +1 and 5 (2.5 %) were + 2. In qualitative CRP test by Omega Kit: 148 (74%) were negative, 32 (16%) weakly positive, 13 (6.5%) +1, 4 (2%) +2 and 3 (1.5%) were +3. A high percentage of Qualitative results, which were weakly positive, became negative by Quantitative methods. The Qualitative results of +1 and the next became positive by Quantitative methods.
Conclusion: It seems that in the early stages of inflammatory disease, quantitative methods are preferred to qualitative methods. Also, in cases that the CRP test results are weakly positive by qualitative methods, they should be controlled by quantitative methods too.

Abstract

Background and Objective: Culture, microscopic method is a gold standard method for identification of Lishmania parasite. The use of Molecular methods such as RT- PCR compared to microscopic methods has a higher sensitivity and specificity however, it is not widely used due to its expensive equipment and the time requested. The use of nucleic acid sequence based amplification (NASBA) method is highly valuable for diagnosis of live parasite because there is no need for to use Thermo cycler. We aimed to assess sensitivity and specificity of NASBA for molecular detection of cutaneous Leishmaniasis.
Material and Methods: First, the RNA was extracted from 28 skin biopsies suspected cutaneous Leishmaniasis. Then, by means of specific primers designed for 18srRNA region, this region was amplified using NASBA isothemal amplification. To increase the sensitivity, the product was electroforesed in TBE (IX) buffer, using Syber Gold Flourecent probes. Using specific primers, RT- PCR was conducted on the samples too.
Result: For diagnosis of Leishmania parasites, NASBA and RT-PCR had the sensitivity of 81% and 51%, respectively, and specificity of 100%.
Conclusion: NASBA isothermal method with high sensitivity and specificity can be applied for identification of cutaneous leishmaniasis.

Abstract
Background and Objective: Nosocomial infection is one of the main factors of mortality in hospitals . One of the most important measures to control infection in hospitals is sampling of equipment and culturing the samples . This study aimed to investigate the prevalence of gram-negative bacilli obtained from the equipment of the hospital wards .
Material and Methods: This Descriptive study was carried out on 488 specimens taken from medical and non-medical devices and equipment of various wards, via census sampling. the samples taken by sterile swabs in Tryptic Soy Broth (TSB) were inoculated in blood agar and Eosin-Methylene Blue agar, and by biochemical standard tests the isolated colonies were identified.
Results: Of 488 isolates, 157 (32.17%) were positive for the presence of gram-negative bacilli. The highest bacterial contamination was related to Enterobacter (37.58%). Of hospital wards, the highest infection related to the surgical wards (19.75%), and the highest contamination is related to laryngoscope blades (12.74%) and ECG sensor device and the monitoring interface (7%).
Conclusion: Microbial infection with gram-negative bacilli is an overwhelming condition in hospital equipment.

Abstract
Abstract
Background and Objective: Agr is the most important regulatory system for the expression of Staphylococcus aureus virulence factors in different conditions. Agr acts as a quorum sensing system in this bacterium which is activated by increased cell concentration during the transition from logarithmic growth phase to stationary phase. Its role is to upregulate the secretory virulence factors such as alpha-hemolysin and inhibit the transcription of surface proteins including protein A-encoding gene. The aim of this study was to assess the relationship between the agr system expression and some virulence factors of Staphylococcus aureus in Brain-heart infusion (BHI) culture medium.
Methods: The expression level of agrA and RNAIII genes from the agr locus along with the expression of hla, spa and mecA genes in BHI broth were assessed in different growth phases using Real time-PCR. Also, gyrB was used as an internal control in this study.
Results: The growth curve of the five tested isolates in BHI broth at 24 hours showed that all the isolates had relatively similar growth patterns. AgrA gene expression in the stationary phase was decreased by 0.89-fold compared with the logarithmic phase. Although the expression of RNAIII gene increased by 3-fold, hla expression decreased by 0.47-fold.
Conclusion: An inactive agr system is observed in the BHI broth medium. BHI broth medium contains high amounts of suitable nutrients for the growth of Staphylococcus aureus, thus the bacteria do not require the activity of the agr system for the regulation of the virulence genes in these conditions.

Background and Objective: Lophomonas blattarum is a multi-flagellate protozoan that is commensal in hindgut of cockroaches and other insects. The protozoan can cause respiratory infection in humans. Most cases of the infections with this protozoan have been reported in China. Here, we present a case with chronic respiratory allergy caused by
L. blattarum in Golestan province, Iran.
Case Description: The case was a 37-year-old male with history of respiratory conditions and he was immunocompetent. An athlete.
L. blattarum was detected in direct smear examination of sputum,
Conclusion: Since we found respiratory infection in an immunocompetent individual who was also an athlete, it is necessary to study this parasite and its life cycle and transmission methods. It is also suggested to consider
L. blattarum infection and treatment with metronidazole in cases of chronic allergies, especially those that do not respond to treatment.

Abstract
Background and objectives: Elevated blood lipids and physical inactivity are known risk factors of atherosclerosis. The objective of this study was to compare effects of four weeks of high-intensity intermittent training (HIIT) and continuous walking on atherogenic indices of obese middle-aged men.
Methods: Study population consisted of 36 male teachers aged 35-50 years, with mean body mass index (BMI) of 30.7 ± 3.5 kg/m2 who were working in the city of Galikesh, northeastern Iran. The subjects were enrolled via purposeful sampling and were randomly divided into two experimental groups and a control group. Before and after the intervention, anthropometric characteristics (height, weight and BMI) and blood pressure of each subject were measured. Fasting blood samples were taken from the left brachial vein 12 hours before the first exercise session and 12 hours after the last exercise session to determine lipid profile. Data were analyzed using SPSS 18 at significance level of 0.05.
Results: The four-week walking exercise significantly decreased serum levels of low-density lipoprotein-cholesterol (LDL-C) (P=0.001) and triglyceride (P=0.001). In addition, the HIIT program significantly increased high-density lipoprotein level (HDL-C) (P=0.004) and significantly reduced LDL-C (P=0.049), LDL/HDL (P=0.002), triglyceride (p=0.01), BMI (P=0.027) and blood pressure (P=0.002). In addition, the results of ANOVA and (Scheffe test) showed a significant increase in HDL-C (P=0.042) values and a significant decrease in VLDL-C (P=0.032), LDL/HDL (P=0.041), triglyceride (P=0.024), BMI (P=0.048) and blood pressure (P=0.016) of HIIT group compared to the control group.
Conclusion: Our findings indicated that HIIT has beneficial effects on some risk factors of atherosclerosis and cardiovascular disease.

Abstract
Background and objectives: Cutaneous leishmaniasis is endemic in most areas of Iran, and the diagnosis of its species is essential for controlling the disease. Leishmania major is the causative agent for cutaneous leishmaniasis in humans. Molecular methods are generally more sensitive than microscopic methods. The present study aimed to use a polymerase chain reaction-enzyme linked immunosorbent assay (PCR-ELISA) technique for detecting live L. major from wounds of patients with cutaneous leishmaniasis.
Methods: In the present study, a standard strain of L. major promastigotes was used as the positive control for purification of DNA. The Novy–MacNeal–Nicolle and RPMI-1640 media were used for reproduction of parasites. DNA was isolated from specimens taken from 35 patients with suspected cutaneous leishmaniasis whose disease was confirmed by direct smear method. The PCR-ELISA technique was later applied by using the standard strain, patient specimens, and primers specific for the 18s rRNA.
Results: Out of 35 patients, 17 (48.6%) were male and 18 (51.4%) were female. In addition, 8.6% of the patients lived in the Gonbad-e Kavus County, while all patients had been infected in villages around Gonbad-e Kavus. Of 35 patients with confirmed cutaneous leishmaniasis according to the direct smear method, 31 patients (86.31%) had leishmaniasis based on the PCR method and the PCR-ELISA methods.
Conclusion: Based on the results, the PCR-ELISA method is more sensitive and accurate for detecting L. major.

Abstract
Background: Leishmaniasis is a vector-borne parasitic disease caused by an obligate intracellular protozoan. Despite the significant frequency of cutaneous leishmaniasis, there is still no appropriate prevention, control and treatment. In recent years, a few studies have been performed on the effects of garlic extract for the treatment of cutaneous leishmaniasis. Current drugs, such as glucantime, have several side effects and lead to direct cellular damage. If an herbal-based drug reduces Leishmania by augmenting the immune system, it has the potential to possess a wider margin of safety. This study aimed to investigate the effects of in vitro systematic review of the effects of garlic (Allium sativum) and its compounds on Leishmania major.
Methods: Five English databases (PubMed, Google Scholar, Embase, Scopus, and Web of Science) were searched until the end of December 2022. The syntax and specific tags have been used for each database. The studies with poor methodology, inadequate information, inappropriate analysis, and confusing presentation were excluded from the current study. The quality of articles was assessed by eleven questions developed by the authors and the data were extracted from the selected articles and discussed.
Results: In this study, 198 articles were selected in the search step, of which five eligible articles were included for examination. The most commonly used solvent for the preparation of garlic extract was distilled water (60%), followed by methanol (40%). In these studies, the bulb of garlic is mostly used (80%). Cell type of J774 was used in only one study (20%), and murine peritoneal macrophages were used in the other studies (80%). The effect of garlic on Leishmania major was strong (80%). Studies have shown that garlic extract or its active compounds can increase cellular immune responses, which play an important role in inhibiting the Leishmania parasite. This is associated with the activation of macrophages and increased IFN-γ levels and NO production.
Conclusion: According to the studies, the effect of different combinations of garlic on the Leishmania parasite has been shown. However, the exact mechanism of the anti-Leishmanial effect of garlic has not been determined. Thus, this issue needs further investigation.
