Abstract

Gastro-enteritis due to Vibrio Cholera was increased suddenly in summer of 1998 in our country. In this research we studied epidemiological and clinical features of Cholerae patients. Vibrio Cholera isolated from 189 (3.3%) patients from 56II stool samples. 182 (96.2%) Vibrio Cholera O1 isolated. 46 (24%) patients with positive stool cultures for Vibrio Cholera were hospitalized. 50% patients were 15-45 yrs. No sex predominency was seen. The important observations in this study including: Fever in 28.1% (13) of patients, sever leukocytosis in 15% (7) of patients, RBC in stool in 33% (15) of patients. Fever and neutrophilia and RBC in stool probably shows co-infection with order intestinal pathogens or different serotypes of Vibrio Cholera with new characteristics. So we are recommending extensive research to be done on serotyping of all of the isolates of Vibrio cholera and other intestinal pathogens simultaneously.

Abstract

Background and objective: Nosocomial infections are the important problems in health care centers in the world. Incidence rate in different countries is very variable and has been reported about 5% to 15%. This research was done for ascertainment of level of knowledge, attitude and practice of employed nurses to nosocomial infection in teaching hospitals of Golestan University of Medical Sciences. Materials and methods: This is a descriptive and analytical study, total number of employed nurses (518), 189 male and 328 female. The research carried out using a questionnaire. The data was used to analyze the data SPSS software. Results: Results of research showed that 26.5% of nurses had very low knowledge, 63.9% medium knowledge and 9.6% good knowledge. About attitude 52.4% had negative attitude, 36.4% medium and 11.2% positive attitude. About practice 37.1% had poor practice, 49% medium and 15.9% good practice. In this research between attitude and knowledge, knowledge and practice there was not any meaningful correlation but difference between attitude and practice was statistically significant. Conclusions: The nurses are playing important role in prevention and control of nosocomial infection. So, it is necessary for nurses to have enough knowledge about how to control the infection and to have positive attitude, if they want to do their job property.

Abstract

Background & Objective: Approximately one-third of the world’s population is infected with Mycobacterium Tuberculosis (TB), which is a indicator of high distribution of these bacteria in our environment. The only vaccine currently available against TB is the attenuated Mycobacterium Bovis strain bacillus Calmette-Guerin (BCG), which used regularly for many years to prevent the Tuberculosis in Iran and many part of the world. The efficiency of this vaccine varies in different populations, and is a matter for discussion. On this basis, the present study has been set up to determine the level of Tuberculin reaction in 4.5 months and in 7 years old children that receive BCG vaccine at birth time, in Golestan province. Materials & Methods: 2700, 4.5 month infant and 2400, 7 years old children in Golestan province were chosen by cluster sampling after the proper permission from the parents, public health centers and educational authorities were taken. The presence of the BCG scar were assessed, and 0.1 ml of 5 tu Tuberculin were injected subcutaneously. The induration was measured 48-72 h after Tuberculin injection. The results were determined as present and compare with T-test. Results: In these study 2559 infants and 2193 child were taken part in the final evaluation. The BCG scar were present in 97.9% of infants and 87.8% of 7 years children, this difference was meaningful. The average induration in 4.5 months babies were 2.29 mm, and in 7 years child was 0.66 mm, this difference was significant (P<0.05). More than 44.7% of babies and 82% of 7 years children didn’t show any reaction after PPD test, this difference was also significant (P<0.05). Conclusion: The level of positive Tuberculin reaction in infants of this province in spite of vaccination was very low, and this level was reduced after 7 years time, this indicate that BCG vaccination at birth did not have any major role in positive Tuberculin reaction. Our results also indicate that the presence of scar can be a good indicator for previous vaccination. In regard to the negative Tuberculin reaction in majority of babies, it is suggested to evaluate the efficacy of BCG vaccine in preventing the TB disease itself, by other method such as studying the TB incidence among children in long term or by determination of cytokines level after Tuberculin injection.

Abstract

A 13 years old female caucasition admitted in 5 Azar General Hospital with a 3-day history of deteriorating vision. 2 weeks prior to admission she had received a Measles-Rubella vaccination at school (As a mass compain of MR vaccination in Iran). General & neurological examination except lung wheezing & ocular findings were normal. In visual exam she had questionable light perception in right eye & hand motion sensation in left eye. Funduscopy revealed moderate bilateral optic disc hyperemia. Investigations revealed normal full cell blood count, biochemistry profile, clotting studies & sputum. Brain CT scan & MRI were normal except pansinusitis. CSF examination revealed normal constituents. The patient treated with Methyl Prednisolone (1 gr/daily) for 7 days and patient had dramatic response to therapy. After 6 weeks of starting medical treatment ophthalmologic exam revealed 2 meters finger count vision in right eye & 6/10 visual aquity in Left side. This patient is a rare case of bilateral optic neuritis after MR vaccination.

Abstract

Background&Objective: Leptospirosis is one of the most common zoonosis in the world,fatality rate of the disease is sometimes high in endemic regions and is transmitted through direct or indirect contact with urine of animals that are carrier of leptospirosis. We studied incidence of leptospirosis in Golestan province in 2004. Materials&Methods: According to coordination with province health center, 20 suspicious patients for leptospirosis were diagnosed by general practioners and referred to 5th Azar hospital. Patients with at least 4 constitutional symptoms (fever, sever headache, conjunctival suffusion, myalgia, arthralgia, icter, malaise) and history of working ir rice farm, exposure to wild or domestic animals or stagnant water were included and MAT for leptospirosis were requested. other causes were ruled out with wright, widal, VDRL, peripheral blood smear. Results: Out of 20 suspicious patients, 12 cases reported. All of them were farmer, had titer>1/100 in MAT. Fever chills, headache were found in nearly all of them. Conjunctival suffusion and increased CPK up to 2 fold than normal was seen in %75 and %66.4 respectively. They were treated by ampicillin or amoxicillin sussessfully. Conclusion: According to the results of this study we suggest to all physician of the area to be concentrated to the disease as an occupational disease in rice farmers. So early diagnosis can prevent many complications of the disease in this group of farmers.

Abstract

Background & Objective: Vaccination is one of the most effective ways in preventing the hepatitis B viruse. This study was done to evaluate the response rate to hepatitis B vaccine in under one-year children in Gorgan, Iran. Materials & Methods: This cross-sectional descriptive study was carried out on 215, children of 7-12 month of age (55.3% male, 44.7% female), in Gorgan, northern Iran during 2006. These subjects already had received the three-time vaccination against hepatitis B. Anti-HBs, Anti-HBc and HBsAg level of these children serum were determined, using ELISA technique. Results: In 30 (14%) subject's serum, there was not detected any antibodies against the viruses. Out of 185 children positive for HBS, 4 cases belong to HBC viruses. As a whole, 86% of the samples, had more than ten international unit of HBS. The meanSD of the titred antibodies in male and female were 158.8412 and 187.5513.83 respectively. The response not to three-times vaccination in male and female were 84% and 89.9% respectively. Conclusion: This study showed that in spit of vaccination, there are cases with virus-contamination. The reasons for absence of any response to the vaccination in some of this children can be due to deficiency in the immune system, improper genetic background which can not be stimulated by that vaccination, inadequate proper conditions for the preservation of vaccine, and the type vaccine used. Also the procedure of vaccine inoculation, the vaccine preservation condition and transportation should be taken into considertion.

Abstract

Background and Objective: Primary TORCH (Toxoplasmosis, rubella, cytomegalovirus, herpes simplex virus) and Treponema palidum infections in the mothers can lead to severe fetal anomalies. This study was done to explore the prevalance of TORCH antibodies in newborns with congenital malformations and their mothers in Gorgan-Iran. Materials and Methods: This descriptive, cross-sectional study was done on newborns with major congential malformations, during 20 months (2003-04) in Dezyani hospital- a referal gynecology center in Gorgan, in north of Iran. The blood sample of 64 newborns with congenital malformations and their mothers collected. Serologic testes were done on newborns and mothers' sera to determine IgM and IgG levels against rubella, toxoplasma gondeie, Cytomegalo virus, Herpes simplex type II with ELISA test. For Terponema palidum PRP test was used. Results: Four of 64 infants (6 %) had positive IgM antibody titers for Toxoplasma, Rubella, and Cytomegalovirus. Nine of mothers with affected newborns (14%) had positive IgM antibody titers for Toxoplasma, Rubella, and Cytomegalovirus. Conclusion: This study showed that the prevalance of TORCH antibodies are observed in 6% of birth defects, in this region.

Abstract

Background and Objective: Abdominal operations as gynaecological procedures result in gastrointestinal dysmotility. Early feeding and ambulation are nonpharmacologic interventions which can be useful in re-initiation of bowel function. This study was done to evaluate the effect of early oral hydration on the return of bowel function and woman's satisfaction after elective caesarean section in primiparous women.

Materials and Methods: In this randomized clinical trial, 120 primiparous women undergone elective cesarean section were assigned to control and intervention groups in Hajar hospital, Shahrekord, Iran during 2007. In the interventional group, oral hydration with liquids was started 4 hours after surgery regardless of presence of bowel sounds and solid food was started after bowel sounds appeared. The control group recieved liquid diet 12 hours after the operation if it was tolerated, they were given soft diet and regular food at the next meal. The return of bowel activity, time of ambulating, satisfaction, discharge from the hospital and complications were compared in two groups. The data were analyzed using SPSS-15, Chi-Square, T and one way ANOVA tests.

Results: The mean postoperative time interval to first hearing of normal intestinal sounds in interventional versus control groups were (9.5±1.38 and 12.5±2.5 hours) the first passage of flatus (15.7±3.61 vs.22.4±4.1 hours), time to first sensation of bowel movement (10.8±1.99 versus 15.7±3.4 hours) and defecation (18.9±3.65 versus 23.4±4.85 hours). These differences were significant (P<0.05). Also discharge from the hospital (0.96±0.18 versus 1.1±34 days) were significantly shorter in interventional group (P<0.05). The women in the early feeding group got out of bed (patient mobilisation) earlier than their interventional group (14.1 hours versus 18.8 hours (P<0.05). Maternal satisfaction was significantly higher among the early fed women (P<0.05).

Conclusion: Early oral hydration after elective cesarean section associated with rapid resumption of intestinal motility and increased woman’s satisfaction.

Abstract

Background and Objective: Since accurate and quick clinical and paraclinical diagnostic methods are not available, in some cases diagnosis of active pulmonary tuberculosis occurs after considerable time from the onset of disease. This study was designed to determine the diagnostic value of High Resolution Computed Tomographic (HRCT) scan in active pulmonary tuberculosis, in Gorgan, Golestan province, North of Iran. Materials and Methods: This diagnostic screening study was carried out on 135 (79 male and 56 female) hospitalized patients suspected with active pulmonary tuberculosis, and HRCT was used in their course of treatment as recommendation of their clinician. The patients were chosen from 5th Azar hosptial during 2009-10. Also it should be mentioned that patients were selected on avaliabity bases, and they were examined by smear, and sputum culture. The patients with negative smear and culture were set up as true healthy group (64 subjects). The lung or small nuddles in HRCT was considered as proper position of lung involvument in active lung pulmonary. The HRCT findings between the case group (71 subjects) and healthy group were compared. According to HRCT findings, the sensitivity and specifity were determined for each patient. Data were analyzed using SPSS-16 and Chi-Square test. Results: In this study, sensitivity, specificity, positive predictive value and negative predictive value of HRCT in active pulmonary tuberculosis were equal to 97.2%, 71.9%, 79.3% and 95.8% respectively. Involvement of upper and middle lobe of the right lung and upper lobe of the left lung were significantly higher than the control group (P<0.05). Conclusion: This study showed that HRCT has high sensitivity and specificity in diagnosis of active pulmonary tuberculosis and can be used as a quick diagnostic way in active pulmonary tuberculosis, especially in patients with strong clinical suspicion and negative smear.

Abstract

Background and Objective: The silent brain infarctions are common in first-ever stroke. Some recent studies show the possible role of silent brain infarction in ischemic stroke. The aim of this study was to evaluate the frequency of silent brain infarction in acute first-ever ischemic stroke and its association with stroke risk factors. Materials and Methods: This descriptive - analytical study was carried out on 203 (94 males, 109 females) patients with acute first-ever ischemic stroke in Shafa hospital in Kerman, Iran during 2010. The diagnosis of ischemic stroke was made by physical examination and was confirmed by MRI and CT Scan. A questionnaire containing demographic information and risk factors including hypertension, diabetes, hyperlipidemia, cigarette smoking, ischemic and non-ischemic heart disease for stroke as well as presence of silent stroke was completed for each patient. Data were analyzed using SPSS-16 and logistic regression test. Results: The mean age of patients was 62.56±17.35 years. Among our patients history of hypertension, hyperlipidemia, cigarette smoking, diabetes, history of ischemic heart disease, history of other cardiac disease and silent stroke were observed in 66 (32.5%), 26 (12.8%), 16 (7.9%), 40 (19.7%), 27 (13.3%), 16 (7.9%) and 31 (15.3%), respectively. The probability of silent stroke among patient with hyperlipidemia, was 3.7 times more than those without hyperlipidemia (95%CI: 1.556-12.780, P<0.05). There was no significant correlation between silent stroke and the above risk factors. Conclusion: Silent stroke was found in 15% of patients with ischemic stroke. Control of hyperlipidemia can have a vital role in the prevention of silent stroke.

Abstract

Borreliosis or recurrent fever is a spirochete disease which is caused by various species of borrelia and characterized by recurrent episodes of fever, chills and spirochetemia that could lead to multiple organ involvement and even death. In this case report, a 23 years old man was presented with shaking chills, fever and abdominal pain from a week before hospital admission. Borrliosis was confirmed following observation of Spirochete in peripheral blood smear. The patient was treated with doxycycline orally and one week after treatment discharged with a good general condition. Coincidence of borreliosis with underlying haemolysis in this patient was misled clinical presentation.

Abstract

Background and Objective: Anti tuberculosis drugs therapy is the most effective method for controling the tuberculosis (TB). Early detection and appropriate treatment can prevent the TB-drug resistance. This study was carried out to determine the complications leading to hospitalization due to consumption of anti-TB drugs in patients with tuberculosis. Methods: In this descriptive-analytic study, 1550 records of patients with TB in urban and rural health centers of Gorgan, north of Iran were assessed during 2007-12. Checklist consists of demographic and clinical data for each patient was recorded in a questionare. Results: 44 cases experienced the complications of anti-TB drugs. 27 (61.4%) of cases with complications were women. 77.3% and 22.7% of patients affected with pulmonary and extra pulmonary tuberculosis,respectively. 38.6% of patients were diabetic. The hepatic complication was seen in 37 cases (84.1%). Skin and other complications were seen in 5 and 2 cases, respectively. There was not any relationship between drug complications and other disases. Conclusion: Hepatic damage is the most common complication leading to hospitalization in tuberculosis patients using anti-TB drugs.

Abstract

Background and Objective: Medicinal drug interactions are one of the problems caused by irrational drug prescription. It eigher change the therapeutic effect or cause drug toxicity. This study was performed to determine the prevalence of medicinal drug interactions in medicinal prescriptions in Golestan province, north of Iran.

Methods: In this descriptive study, 1100000 medicinal prescriptions under contract with the Golestan province health insurance organization were assessed for drug interactions during 20 March-
20 September 2012.

Results: Drug interactions frequency was 6262 (0.66%). 35.5%, 63.1% and 1.4% of prescriptions were severe, moderate and mild interactions, respectively. The most common severe, moderate and mild drug interactions related to Atorvastatin-Gemfibrozil (13.67%), Ceftriaxone- Gentamicin (9.05%) and Lithium Carbonate-Haloperidol (2.56%).

Conclusion: In view of moderate and severe medicinal drug interactions in physicians' prescriptions, health system should plan a comprehensive program to improve awareness and effective monitoring to reduce medicinal drug interactions.

Abstract

Background and Objective: Nursing is one of the most stressful occupations and one of the common stress-induced syndromes among nurses is low resilient. Raising resilience is one of the effective factors in dealing with tensions. This study was done to determine the relationship between optimism and humor with resilience in female nurses.
Methods: This descriptive-analytic study was carried out on 230 female nurses who worked in Isfahan, Iran during 2016. The number of nurses in different departments in these hospitals was 600; finally, 235 people were randomly selected using the Morgan table. In order to collecting data, sense of humor questionnaire by Lefcourt & Martin (SHQ), life orientation optimism questionnaire Carver & Scheier (LOT-R), and Conner and Davidson resilience questionnaires were used.
Results: There was a significant negative relationship between optimism and resilience (P<0.05). Also, there was a significant and positive correlation between humor and resilience (P<0.05). The fact that all the sub-scales of humor can predict resilience, laughter has the greatest effect on resilience.
Conclusion: This study showed that with increasing humor, resilience is rising in female nurses.

Abstract

Background and Objective: There is a corolation between meta-cognitive beliefs and psychological adaptability. The subjects with adaptability beliefs have more soci-psyclogical adaptability. Diabetes has a negative impact on the quality of life. Therefore, this study was done to determine the relationship between meta-cognitive beliefs and adaptability with quality of life in type 2 of diabetic patients.
Methods: This descriptive correlation study was done on 100 type 2 diabetic patients (60 males, 40 females) whom refered to association of diabetes center in Tehran, Iran during 2016. They were selected by network sampling method. Data collection was done by Wales and Cartwright Hatton (2004) Meta-cognitive beliefs, Bell's (1961) adjustment and ware and quality of life (1996) questionnaires.
Results: There was a positive and significant relationship between metacognitive beliefs (r=0.756) with quality of life (P<0.05). In addition, there was a significant relationship between adaptability (r=-0.917) with quality of life (P<0.05). The results of sub-scales of metacognitive beliefs showed that the strongest predictor of adjustment and quality of life is the component of positive beliefs about worry, and then the components beliefs about superstition, punishment and responsibility associated with worry and beliefs about uncontrollability of worrying and about the dangers of failing to control worrying, respectively, had a good predictor role.the severity of diabetes significantly reduced the qulity of life (P<0.05).
Conclusion: Metacognitive beliefs and compatibility can affect the quality of life in diabetic patients.

Abstract

Background and Objective: Trauma is the most important causes of lossing effective life in human comunities, and it is the second leading cause of death in Iran. This study was done to determine the pattern of penetrating injuries and intrathoracic organ damage in Gorgan, north of Iran.
Methods: In this descriptive study, data of 107 patients with penetrating injuries whom referred to
5th Azar hospital in Gorgan-Iran during 2016-17 were collected. Age, gender, job, type, location and cause of injury were recorded for eacgh subject.
Results: The mean age of patients was 26.13 years. The most frequent job was related to unemployed group (39.25%). The most freqent causes of trauma were quarrel with 78.5%. The most divice were knife with 66.35%. Lung was the most injured organ with 11.21%. The least damage was to the large vessels with one case (0.93%). The two patients died due to heart and subclavin vessels injuries.
Conclusion: This study showed that injury due to knife was the most prevalent of penetrating trauma in street strife in northern Iran.

Abstract

Osteoid osteoma is benign bone tumor, in which talus bone involvement is rare. This case was a 17 year old man with a right ankle pain and reduced range of motion since one year ago. Following clinical evaluation and radiological studies (X-ray, CT scan, and MRI), a bone tumor was detected in the talus with a possible diagnosis of Osteoid osteoma. The patient was operated with ankle anterior approach and the tumor was resected from the site. The patient was discharged after two days. The patient's pain was eliminated after surgery. Ankle movements are back after two months. Due to the rare nature of the disease and clinical diagnostic problems and its unusual radiographic manifestations, osteoid osteoma in chronic ankle pain in young patients, is included in the list of differential diagnosis.
