Abstract

Background & Objective: Post-general anesthesia urinary retention is a common and bothering complication. In this study prediction value of IPSS for post-general anesthesia urinary retention was evaluated. Materials & Methods: This prospective study include 100 male patients over 50 years old who were candidated for nenurologic and non-nenurological elective surgery without any urology and neurologic past medial history. IPSS was calculated for each patient. The patients were followed in post operative period for urinary retention. Results: 1.8% of patients who had mild symptoms (IPSS=0-7) had urinary retention in 2nd 24 hours of post-operative period, 20% of patients who had IPSS=8-19 and 53.3% of patients with IPSS=20-53 had urinary retention at same period (P<0.05). Conclusion: IPSS can predict postoperative urinary retention in elderly male patients. If severe symptoms by IPSS was encountered the patients and health care personals must be cautioned about possibility of postoperative urinary retention and if required urologic consulation.

Abstract

Background and Objective: Success rate of IVF depends on infertility factors and its treatment cycle. Different methods of treatment and different centers have reported various rates of success. The aim of this study was to evaluate the fertility outcome after IVF and related factors. Materials and Methods: This descriptive analytic study was performed on 150 infertile couples following IVF in Montaserieh Infertility Center, Mashhad University of Medical Sciences, Mashhad, North – East of Iran during 2001-06. Data was collected by a questionnaire including woman's age, husband's age, infertility factor, spermogram, infertility period, the number of follicles caused by treatment, the number of oocytes, the number of produced and transferred fetus and treatment outcomes. Data was analyzed by SPSS software and Chi-square, t-studen test, One-way ANOVA and logistic regression. Results: The mean age of women was 29.7±5.4. The mean±SD follicles oocytes and transferred fetus were 11±6, 6±3.9 and 2.6±1.5 respectively. There was significant relation between the number of follicles (P<0.05), obtained oocytes (P<0.05) and transfered fetus (P<0.05) with the success rate of IVF. The fertility rate was 24.6%. There was no significant relation between the age groups and fertility rate. Conclusion: This study showed that ovarian response to ovulation stimulation and the number of transferred fetus are the important and effective factors for predicting the outcomes of IVF.

Abstract

Background and Objective: Cardioembolic stroke account for one-fifth of ischemic stroke and atrial fibrillation is the most common underlying cause. Taking an oral anticoagulation (Warfarin) is an effective way of preventing ischemic stroke but bleeding complication is common. This study was carried out to evaluate the validation of HASBLED score in prediction of hemorrhagic complications in patients with brain ischemia and atrial fibrillation under warfarin therapy. Methods: In this cohort study 112 patients with non-valvular atrial fibrillation in term of major and minor bleeding complications were followed to the predictive value of HASBLED criteria for one year. Major bleeding complications defined as intracranial bleeding, bleeding leading to hospitalization, drop of hemoglubin of more than 2gr/dl or requiring transfusion. HASBLED criteria were defined as hypertension, abnormality in liver and renal function tests, history of stroke, history of bleeding, large fluctuations in coagulation tests results, age more than 65 years and an Anti-platelet and non steroidal anti-inflammatory drugs and alcohol use one point is awarded to each of the aboves. Results: During one year follow up, 10 (9.1%) of patients had major bleeding and 28 (25%) patients had minor bleeding. The risk of major bleeding was significantly related with a history of minor bleeding and HASBLED scores (P<0.05). The risk of minor bleeding was significantly related with warfarin toxicity and high INR (P<0.05). HASBLED score>3 was associated with the likelihood of major bleeding in future. Conclusion: Patients with HASBLED score>3 should be causious in initial stage of taking oral anticoagulant.

Abstract

Background and Objective: The rise of antibiotic resistance particulary Methicillin resistance in pathogenic bacteria such as Staphylococcus aureus is found to be an emerging threat to human health especially in hospitals. Heavy metal nanoparticles such as Ag used for inhibition of this bacterium. This study was done to determine of minimum inhibitory concentration (MIC) Ag nanoparticle against Staphylococcus aureus which isolated in Gorgan, north of Iran and its relation with Methicillin resistance and source of bacteria.

Methods: In this descriptive – analytical study, the MIC Ag nanoparticle in 183 isolates of Staphylococcus aureus by microdilution method was determined. 30 isolates, based on mecA gene was considered as MRSA. Samples were collected from patients, nose of healthy carriers and foods. Compare the MIC of isolates based on Methicillin resistance, source of the bacteria and resistance to other antibiotics were assessed.

Results: Out of 183 samples MIC was varied from 1 to 16 µg/ml, and mean±std was 2.9±1.89 µg/ml. MIC mean of silver nanoparticles in isolated from foods were 2±0.7, isolared from healthy carriers were 4.1±2.4 and from patients were 3.4±2.1 µg/ml and were statically significant (P<0.05). MIC mean of silver nanoparticles in MSSA isolates are 3.9±2.3 and in MRSA isolates are 2.4±1.4 µg/ml that were statically significant (P<0.05). MIC mean of gentamycin resistant isolate were lower than sensitive one. But between MIC of silver nanoparticles and other antibiotics resistance was not significant statistically.

Conclusion: There is a relation between silver nanoparticle MIC, source of sample isolation, Methicillin and gentamycin resistance. Since MIC of silver nanoparticles on isolates of Methicillin resistant is low, the possibility of its use in the control of MRSA in hospital infections can be considered as a prime attention the Gentamycine.

Abstract

Background and Objective: Chronic infection with Hepatitis B virus (HBV) is one of the main causes of cirrhosis and hepatocellular carcinoma (HCC). The pathogenicity of the virus is determined by the multi-functional protein x (HBx). Changing the sequence of the gene encoding this protein causes the regulation of transcription and pathogenicity factors. This study was done to analyze the genetic dynamics of the HBx coding gene in a person with chronic HBV.
Methods: In this descriptive laboratory study, an infected person with chronic hepatitis B virus infection was first amplified and cloned into complete sequence of HBx encoder. Then, the reference sequences of genotypes, serotypes and different virus subtypes of the GenBank database were matched by CLC Sequence Viewer software. The comparative result was used to plot the phylogenic tree by T-rex server and population genetic analysis using DnaSP software. Natural selection at the nucleotide and protein level was performed by the Tajima's D test.
Results: No known mutation at the level of the protein was found in the chronic sequence of the HBx encoder. The results of natural selection indicated neutral mutations in the HBx gene. The phylogenetic results showed that the HBx encoding sequences in the chronic infected individual had a genetic affinity with genotype D and ayw2 subtype.
Conclusion: Neutrality polymorphism takes place in HBx coding region. Also, the phylogenetic results of the present study are consistent with the previous findings of Golestan province and Iran which have reported the prevalence of genotype D and subspecies ayw2.

Abstract

Background and Objective: Life style with Low activity and overweight increased the risk of cardiovascular diseases. This study was done to evaluate the effect of ginger supplementation on Lactate dehydrogenase (LDH) changes and delayed muscle soreness following exhaustive eccentric exercise on overweight girls.
Methods: In this randomized clinical trial study, 22 overweight girls were randomly divided into the two groups including intervention (n=12) and control (n=10) groups. For subjects in interventional group were used a daily supplement of 2 grams of ginger powder, each serving a capsule, containing one gram of ginger powder  and subjects in control group were received  daily two capsules containing starch, for two weeks. Physical activity was performed consisting of two sessions of exhausting activity (negative tilt on treadmill, one session before supplementation and one session after two weeks of supplementation). Blood samples were measured in order to measure changes in LDH in four stages and the use of the pain questionnaire for evaluation of delayed muscular brittleness in both pre and post gingival supplementation. Pain scale (visual analogue scale: VAS) was used to measure the delay fatigue.
Results: The results showed that extrinsic exercise increased LDH and delayed muscle soreness after exercise (P<0.05). Also, taking two weeks of ginger supplementation significantly reduced LDH and delayed muscle soreness in the supplement group in comparison with controls (P<0.05).
Conclusion: Ginger can reduces the muscle gain and delayed onset muscle soreness resulting from eccentric exercise in overweight girls.

Abstract

Background and Objective: Periodontal disease has high prevalence in diabetic patients and the knowledge of them about the relationship between diabetes and periodontal disease is effective in the control of the blood glucose and progression of periodontitis. This study was done to evaluate the knowledge, attitude and practice of diabetic patients about periodontal disease in Gorgan north of Iran.
Methods: This descriptive-analytical study was done on 300 type 1 and 2 diabetic patients (136 males & 164 females) whom referred to governmental health centers in Gorgan, northern Iran during 2019. Demographic information, knowledge, attitude and practice were recorded using a questionnaire.
Results: The level of good knowledge, attitude and practice were evaluated 51.7%, 44.3% and 25%, respectively. There was significant association between oral hygiene practice and gender and education (P<0.05). Good practice was observered more in females and in patients.
Conclusion: More than half of diabetic patients had moderate to good knowledge and attitude about periodontal disease but their practice was moderate to poor.

Abstract

Background and Objective: Obesity and overweight, one of which symptoms is chronic inflammation, is associated with changes in tryptophan metabolism, culminating in numerous diseases. Sport training is among the ways to control obesity and overweight. This study was conducted to determine the effect of eight weeks of endurance training on amino acid tryptophan serum levels and body mass of overweight women.
Methods: This quasi-experimental study was conducted on 24 overweight women in two groups of 12 people, including the control and experimental groups. The mean age and standard deviation of the control and experimental groups were 41.16±3.45 and 41.08±2.93 years, respectively. The body mass was measured using the anthropometric device, and blood samples were taken in the pre-test and post-test in a 12-hour fasting condition. The subjects of the experimental group underwent endurance training 3 sessions a week for 8 weeks with an intensity of 60% to 75% of the maximal heart rate reserve. Serum tryptophan level was measured by enzyme-linked immunosorbent assay (ELISA) method.
Results: Eight weeks of endurance training led to enhancing serum tryptophan levels, reducing fat mass, and reducing body mass index (BMI) (P<0.05), and also resulted in a statistically insignificant increase in fat-free mass in overweight women.
Conclusion: Eight weeks of endurance training could be effective in improving the physiological status and body composition of overweight women.
