فراوانی ژن‌های blaZ و mecA در ایزوله‌های استافیلوکوکوس اورئوس جدا شده از نمونه‌های بالینی با روش PCR

زمینه و هدف: استافیلوکوکوس اورئوس شایع‌ترین عامل مهم عفونت‌های بیمارستانی است. درمان عفونت‌های استافیلوکوکوس اورئوس با حضور گونه مقاوم به آنتی‌بیوتیک متی‌سیلین (Methicillin Resistant Staphylococcus aureus: MRSA) پیچیده‌تر شده است. mecA ژن کدکننده مقاومت به متی‌سیلین و blaZ ژن کدکننده مقاومت به آنزیم بتالاکتاماز است. این مطالعه به منظور تعیین فراوانی ژن‌های blaZ و mecA در ایزوله‌های استافیلوکوکوس اورئوس جداشده از نمونه‌های بالینی با استفاده از روش مولکولی واکنش زنجیره‌ای پلیمراز (Polymerase Chain Reaction: PCR) انجام شد.

روش بررسی: در این مطالعه توصیفی – تحلیلی 59 ایزوله استافیلوکوکوس اورئوس از نمونه‌های بالینی بیمارستان‌های شهر گرگان طی مدت شش‌ماه (از بهمن 1395 لغایت تیر 1396) جمع‌آوری شدند. ایزوله‌ها با استفاده از رنگ‌آمیزی گرم، تست‌های کوآگولاز، کاتالاز، Dnase و تخمیر قند مانیتول تعیین هویت شدند و با روش دیسک دیفیوژن مقاومت آنتی‌بیوتیکی ارزیابی گردید. از روش یدومتری برای تشخیص تولید آنزیم بتالاکتاماز در این باکتری استفاده شد. سپس تشخیص ژن‌های mecA و blaZ با استفاده از روش PCR انجام گردید.

یافته‌ها: همه 59 ایزوله (100 درصد) استافیلوکوکوس اورئوس برای ژن blaZ مثبت و به‌دنبال آن، 27 ایزوله (45.8%) دارای ژن mecA بودند که به‌طور همزمان ایزوله‌های دارای ژن mecA از لحاظ حضور ژن blaZ مثبت بودند. میزان 5 درصد از ایزوله‌های مقاوم به اگزاسیلین و 3 درصد از ایزوله‌های مقاوم به سفوکسی‌تین دارای ژن mecA بودند. 79.4% از ایزوله‌های دارای ژن blaZ، در تست فنوتیپی بتالاکتاماز مثبت بودند.

نتیجه‌گیری: این مطالعه نشان داد که تمام ایزوله‌های جدا شده دارای استافیلوکوکوس اورئوس با ژن blaZ بودند و به‌طور همزمان ایزوله‌های دارای ژن mecA از لحاظ حضور ژن blaZ مثبت بودند.
