Documents required to be sent for graduate students to the Student Affairs Center

 

 ( A ): Completing the academic records that include the following conditions:

 
1 - Photocopy of birth certificate [ a complete series of all pages ]
2 - Photocopy of ID Card
3 - High school Diploma
4 – Certificate of the Freshman year
5 - Transcripts of the three-years of diploma
6 - Verification of freshman and diploma certificate
7 – Exam entry card
8 – Checking the military service Status
9 - Photo
10 - Completed the obligation form (free education)
11 – The Form of Central Code

12 - Transcripts sent to the Department of Education School of Education

13 - Sending the form of student loan situation from the student and culture deputy to the Department of Education Affairs
14 - Evaluating the student graduation status regarding education probation, prerequisites, withdrawals, and check if the concern student has already taken all courses related to her/his academic major regards the academic standards.

 15 - Completing the graduation form by the academic experts of the university and approval from the education deputy manager and sending to the Ministry of Health and Education

 

  (B) Issuing a temporary certificate for graduated students

 
1 – Temporarily confirmation of the graduation certificate with is a confidential correspondence in the university support deputy

 2 - Temporarily confirmation of the graduation certificate takes at least two working weeks after the graduation form is completed and issued to the Ministry of Health and Education.

 

 ( C ): Documents needed for a temporary graduation certificate:

 
1 - Valid documents of the student internship and the internship exemption
2 – Clearing the student loan situation from the Medical Students' Welfare Foundation
3 – Exemption from military service for male students
4 – Paying Rls 10,000 to the bank account number 14/108 to the Central Bank of Iran
5 – The temporary graduation certificate is valid for 5 years and is extendable to one additional year.

   
Documents required for issuing graduation certificate:

 

 1 - Original documents of the student internship and the internship exemption
2 – The evidence of student loan clearance
3 - Original documents of exemption from military service for male students
4 - The permanent license
5 - Certificate of work done inside the country 
 6 - Graduation certificate from the previous university
