Abstract

Due to importance of menopause and it’s effects on the menopausal women, and also the use of hormonal replacement therapy, we decided to evaluate side effects of Estrogen therapy on menopausal women, such as sleep disorder and related problems. 60 menopausal women entered a 3-months prospective, placebo-case controlled, double blind and accidental study. They were selected after taking history and physical examination, some necessary laboratory data, exclusion of interfering factors. They were divided in 2 groups with equal members. Conjugated Estrogen (0.625 mg per day) was given orally for 3 months to case group. Placebo (1 tablet per day) was given to control group. The 2 groups evaluated again after 3 months. A questionnaire with 7 questions was given to the members of 2 groups before and after treatment. Treatment with Estrogen reduce number of sleep awakening, daily drawsiness, morning tiredness and improve the sleeping and resleeping (P<0.05) and with confidence interval of (95%). In our study, there was not significant correlation between weight, age, duration of menopause, cause of menopause with sleep disorders. According to this study, we conclude that Estrogen replacement therapy reduce sleep disorders in post menopausal women. These findings confirm HRT benefits.

Materials and Methods: This descriptive cross sectional study was done on 105 patients (76 female and 29 male) suffering from osteoporosis, evaluated in the endocrinology Department of Sina hospital, Tabriz- Iran from March 2003 to March 2006. Past medical history clinical symptoms and biochemical results were of patients. Data analyzed using SPSS-14  and chi square test.

Results: Osteoporosis and osteopenia were seen in 55% and 45% of patients with reduced bone density, respectively. Daily calcium intake in patients with less than 400 mg, between 400-1000 mg and more than 1000 mg were 63.8%, 31.9% and 3.4%, respectively. The mean±SD of sera calcium and vitamin D level were 9.5±0.6 mg/dl, 45±37.1 nmol/l respectively. 61.2% of patients had vitamin D deficiency. 33% of patients had secondry osteoprosis. Among the patient with primary osteoprosis 11.3% afflicted to hyper claciuria.

Conclusion: This study showed that decresing bone density was more prominate in women. The rate of daily calcium intake among patients were low. It is sugested these patients osteoprosis could be prevented by consumption food nutrient rich in calcium and vitamin D suplementation.

Abstract

Trazodone is a weak inhibitor of serotonin reuptake and a potent antagonist of serotonin 5HT2A and 5HT2C receptors. Urinary incontinence is a rare side effect of anti-depressant drugs. Side effects of antidepressants can cause delay in treatment of patient and ultimately non-compliance in patients who take this medicinal drug. Here we present a case of trazodone induced urinary incontinence in a woman at menopause period. The patient was a retired woman with 63 years old. She reported a history of bladder prolapsuse and history of consumption of quetiapine and propranolol. But the patient had no history of other somatic illnesses, drug and urinary incontinence before administering the medicinal drug. Psychiatrist prescrided trazodone with dose of 50 mg, orally per day when appearence of the depression, anxiety and insomnia. But, the patient suddenly suffered from urinary incontinence, despite this situation the patient had continued taking the drug for three days. Due to the uncertainty of the above complications patient, was forced to discontinue the drug and follwoing drug discontinuation the side effect disappeard. After the improvement of the urine incontinence and full recovery, re-treatment was began with the same dose of 50 mg trazodone for the patient, but immediately after drug administration to the patient, urinary incontinence appeared once again. Trazodone can be considered as important factor to cause urinary incontinence. Therefore, this issue should be considered whenever this pharmaceutical drug is prescribed.

Abstract

Background and Objective: The comorbidity of type 2 diabetes mellitus (T2DM) and hypertension may exacerbate chronic inflammation and regular exercise training considered as an important therapeutic approach for such patients. This study was done to evaluate the eight weeks effects of regular exercise training on apelin and tumor necrosis factor alpha (TNF-a) plasma levels in hypertensive postmenopausal women with T2DM.
Methods: This quasi-experimental study was conducted on 20 postmenopausal women with hypertension and T2DM. Subjects non-randomlly allocated into intervention and control groups. Training program consisted of 25-40 minutes of aerobic exercise at 50-70 percent of maximal heart rate, 3 sessions per week for 8 weeks. The plasma level of apelin, TNF-a and glucose was measured, subsequently.
Results: 8 weeks exercise training was significantly reduced apelin, TNF-a, and glucose levels in the intervention group compared to the control group (P<0.05).
Conclusion: Aerobic exercise can mediate some of its favorable effects on hypertension pathological conditions associated with type 2 diabetes mellitus by reducing plasma apelin and TNF-a levels.
