Abstract

The treatment of supraventricular arrhythmia include wide range of medical intervention, but ideal drug for treatment of this kind of arrhythmia is yet to be developed. Pharmacological interventions due to their adverse side effects and the possibility of proarrhythmic effects are usually ineffective in treating these conditions. Herbal remedies are suitable alternatives for synthetic drugs, due to their availability, minimal side effects and lower price. Pharmacological studies and traditional medical literature point to the cardiovascular effects of the Achillea Santolina in many instances. In present study, we used isolated heart of langandrof rats as an experimental model to determine the effect of various concentrations of the methanol extract of Achillea santolina (2×10^-7, 2×10^-9, 2×10^-10 W/V) on the electrophysiological properties of the isolated heart. Results of this study show a significant depression of WBCL, AVCT and ERP and non-significant increased in time constant of recovery (trec). It may be therefore be considered a potential role for anti-arrhythmic effect of Achillea Santolina in suppression or treating supraventricular tachyarrhythmia.

Abstract

Background and Objective: Heart failure is the end stage of cardiac disease after that myocardium has used all its reserve and compensatory mechanism. This is a descriptive survey for determine underlying and precipitating etiology of congestive heart failure among patients who admitted on CCU and cardiac department of 5th Azar Hospital of Gorgan. Materials and Methods: In this study 145 patients aged between 19-87 years have participated by data collection using questionnaire and analysis by SPSS-10. Results: The results indicated. The most common underlying causes are myocardial ischemia (57.3%) and the most common precipitating factors is inappropriate drug therapy (75.8%). Orthopnea (86.2%) and ralls (82.8%) are the most symptoms and physical exam findings. AF rhythm with 24.1% is the most common arrhythmia and cardiothoracic ratio more than 0.5 is the most abnormal finding in CXR-PA. The average ejection fraction (EF) in male patients was 32.3% and in women it was 35.7%. There was meaningful relationship between male sex and EF (P<0.05). Conclusion: This study indicated that good control of HTN and preventing of (CAD) incidence are the most important factor to avoid from CHF and appropriating drug therapy has the essential role to maintain compensated state in heart failure.

Abstract

Background & Objective: Cardiovascular disease is among important factors of mortality in hemodialysis patients. There are some reports that indicate the Carnitine concentration reduction in the tissues accelerate cardiovascular abnormalities in those patients whom regularly are hemodialysed. This research has been set up to study the effect of oral Carnitine on the heart function of hemodialysis patients from Sari and Vali-Asr Hospital in Ghaem-Shahr Fatemeh Alzahra, Imam Khomeini in during 2003. Materials & Methods: In this study 20 patients with at least 3 month duration and 3 times interval hemodialysis in each month were chosen. The sample hemogenously devided in case and control group. One Carnitine tablet with 1 gr dose was given to the case group daily. The placebo was prescribed to the control group EF and LVEDD of patients were determined by echocardiography. The ratio diameter of heart to the chest was determined by chest radiography. The plasma level of hemoglobin, Cholesterol and Triglyceride were determined simultaneously. Results: There was not any meaningful recovery in EF and LVEDD and serum Lipids, between the case and control group 6 months after treatment with 1 gr/day Carnitine. There was only a meaningful difference in EF after 6 months duration of this study. In spite of this, there was a meaningful recovery with these patients anemia (P?0.05). Conclusion: Carnitine does not have a meaningful effect of heart function and serum Lipid level, but it has an efficacy on anemia recovery of such patients.

Abstract

Background&Objective: The main cause of the cardiovascular disease is atherosclerosis of coronary artery. One of theraputic methods of the disease is coronary artery bypass graft (CABG). One of the main complicatons of CABG is bleeding after grafting. Different methods and proposed for preventing or reducting the bleeding. The aim of this study is to evaluate the effects of tranexamic acid (TA) on reduction of bleeding after grafting.

Materials&Methods: This study was a randomized clinical trial. 100 patients conditated to CBG divided into 2 groups randomizely (50 for control and 50 for subject). TA was injected to subject group twice with the same dosage of 15 mg/kg: first during the operation and second after ending the cardiopulmunary pomp and neutralizing the theraputic effect of heparin by protamin. The same method was used for the control group, except normal saline was used instead of TA variations such as bleeding rate, PT, aPTT were tested after CABG. The extracted data was analysed by SPSS software.

Results: The range of ages was 28 to 75 years and the control and subject groups were distributed normally from age viewpoint. Bleeding rate after grafting in subject group was 335±45 ml and in control group was 490±81 ml. in case and control platelet count and hematocrit groups, were significant differences (P<0.05).

Conclusion: Using TA during and after CABG is suggested to control bleeding.

Abstract

Background&Objective: Dyspnoea and fatigue caused considerable impairment in the functional performance and quality of life in HF patients. The purpose of this study were to determine efficacy of home walking exercise program on functional performance and quality of life in patients with heart failure. Materials&Methods: This study was qusiexperimental trial that assessed efficacy of home-based exercise program on the functional performance and quality of life in patients with HF in the Ahvaz city (2005). In this study 60 patients with New York Heart Association (NYHA) class II and III heart failure divided two groups training (n=30) and control (n=30). Material or measurements was demographic characteristics form, Minnesota quality of life check list and timed exercise program form. Exercise training in the patients would be performed tree day per week for 8 weeks. Determination quality of life measures by Minnesota check list would be performed in both the training and control groups at entry and after 8 weeks. Also 6 minute walking tests for determination functional performance would be performed in both groups at entry and after 8 weeks. Ultimately data analysis by SPSS softward. Results: results showed that significant difference existed between mean walking distance on the 6MWT at entry and after 8 weeks in the training group (373.86 to 412.30 m, P<0.05), that no significance was seen between control group (376.79 to 377.63 m). Also significant difference exists between mean quality of life scores at entry and after 8 weeks in the training group (52.32 to 43.80), that no significance was seen between control group (52.43 to 52.50). Conclusion: This study showed that home-based exercise program affected on functional performance and quality of life in HF patients. Its accepted. Therefore exercise training can be used as a therapeutic approach in these patients, because not only promotes quality of life but also improve the functional performance.

Abstract

Background & Objective: Congenital spinal deformities usually present in infancy and make parents  worry about their child future. Because of congenital nature of these deformities, cord anomalies and other organ abnormalities must be evaluated. This study was done to determine Congenital spinal deformity and associated anomalies.

 

Materials & Methods: This cross sectional study was done on 97 patients who referred to Shafa hospital in Tehran- Iran with congenital back deformities from October 2005 to January 2007. All patients were assessed with physical examination, spinal radiography, MRI, urinary system sonography and echocardiography.

 

Results: Mean age of patients at presentation was 81 month. 81 patients had congenital scoliosis and 16 patients had congenital kyphosis. Mean cobb angle was 52 degrees for congenital scoliosis and 70 degrees for congenital kyphosis. 22 patients had neurologic or skin signs that 81% of them had cord abnormalities (p<0.05). Cord anomalies were seen in 36 scoliosis (46.5%) and 2 kyphosis (15%) (p<0.05). Most common cord anomalies were syingomyelia,diastematomyelia and tethered cord. Congenital heart anomalies was seen in 3 patients (4%) and valvular heart disease in 10 patients (13.5%) and kidney anomalies in 7 patients (12%). Only one patient with congenital heart disease and one patient with kidney anomaly had kyphosis.

 

Conclusion: Regarding to prevalence of congenital spinial deformity (CSD) in scoliosis and kyphosis patients, all of patients with CSD should screued for determiation of conganital anomalies by MRI, Eco cardiography and kidny sonography.

Abstract

Background & Objective: The atherosclerosis of left anterior descending (LAD) coronary artery because of subtending more than 50% of left ventricular myocardial mass, has long been the focus of concern regarding appropriate treatment. This study aimed to evaluate efficacy of Percutaneous Transluminal Coronary Angioplasty (PTCA) with stenting for treatment of proximal LAD disease. Materials & Methods: In this descriptive study, 75 patients with isolated proximal LAD atherosclerotic lesion treated with PTCA and stenting were followed up for 9 months. The stenosis of all patients was documented by coronary angiography prior to enrollment. Exercise tolerance testing (ETT) by Bruce protocol was performed before angioplasty and at the end of follow up period as well. Results: There were 44 (58.7%) males, and 31 (41.3%) females. The mean age of patients was 59.4 (range, 39 to 83 yrs). Clinical outcomes of 73 (97.3%) patients were satisfactory. Only two patients developed re-stenosis during the follow up period. The first patient developed myocardial infarction at the 3rd day of her admission, and the second one had an abnormal ETT after 9 months. Percutaneous coronary revascularization with stenting was performed, and both of them were discharged without any problem. Conclusion: This study demostrated that the nine months prognosis of patients with isolated stenosis of proximal LAD coronary artery that treated by PTCA and stenting was acceptable. This treatment improves clinical status and results in lower hospital adverse event rates and in appropriate clinical outcome.

Abstract

Background and Objective: Electromagnutic field of cell Phones may affect the biological systems. This study was done to evaluate the effect of electromagnutic field of mobile phones on blood pressure, heart rate and arytmia on students of Kurdistan University of Medical Science, in West of Iran. Materials and Methods: In a randomized, double-blind clinical Trial, 56 young healthy volunteers with 20-30 years age were divided in two groups: expousure (n=27) and control (n=29). Cell phone was kept in front of the heart. Non functional cell phone was used in the control group. Systolic and diastolic blood pressure, heart rate and basic ECG were recorded. The Parameters were recorded in stage one (before set on cell phone), stage two (the first 5minutes), stage three (6minutes,with silent ringing), stage four (after 17min conection) and stage five (after 35 min conection). In all Stages, it was evaluated by cardiac monitoring (Lead2) for the presence of (sinus arryhthmia, sinus bradycardia, SA block, AV block, PAC and PVC). Results: Systolic and diastolic blood pressure and heart rate did not show to have any significant differences between two groups in any stages. Rate of sinus arryhthmia was higher in expousure group at stages four and five, but only in stage four, this difference was shown to be significant (P<0.05). Conclusion: The findings of this study showed that, electromagnetic of cellular phone cause sinus arryhthmia after 17 minute connection, to prevent the side effect of cell phone, we suggest that, cell phone set distant away from heart and reduce the connection time.

Abstract

Background and Objective: The modern life condition has caused the human to be exposed to electromegnetic fields. Epidemiologic and animal laboratory studies have illustrated the adverse effects of electromegnetic fields on biologic systems. The aim of study was to assess the effects of low density electromegnetic on heart tissue of male rat. Materials and Methods: In this experimental study 30 Wistar rats with weight of 10-150 gram and 5weeks age were used. Rats were allocated randomly in two aqual groups: case and control. Experimenal group rats were exposed to the electromegnetic field 8 hours a day for 2 months (50 HZ, 1 mili Tesla). After 2months the rats were sacrified, the heart tissues were removed and stained by H&E method. Results: There was not any altherations in cells and tissue of control groups. In experimental group there was a sever disruptions on heart tissue, polymorphysm of cardic myocyte nuclues with same hollow spaced in perinucluar, nuclear cytoplasem and fibrotic tissue. Also, the number and size of cells in case group were reduced. Cagulated necrosis and vacolization of cytoplasm of cardiac myocyte was observed in case group. Conclusion: This study showed that low density of electromegnetic Fields for two months durtion can cause pathological changes in cells and heart tissue of male rats.

Abstract

Background and Objective: Electroconvulsive therapy (ECT) is one of the most common methods in treatment of different types of psychological disorder. The effectiveness of this therapy has a direct relation with the duration of convulsion. This study was conducted to assess the effect of lidocaine on duration of seizure and hemodynamic alterations in electroconvulsive therapy. Materials and Methods: This clinical trial study was conducted on 72 ASA-I, II patients with psychotic disorders in Hajar Medical Center in Shahrekord, Iran during 2010. The patients randomly divided into intervention and control group. The interventional group was received 1.5 mg/kg lidocaine and controls were received normal saline. For induction of anesthesia, all patients were received Sodium Thiopental (2mg/kg), Succinylcholine (1mg/kg) and Atropine (0.5mg) Propofol and Succinylcholine during 72 sessions of ECT. Duration of objective convulsion and hemodynamic alterations including blood pressure and heart rate were recorded (before, immediately and 3, 5 minutes after ECT). Data were analyzed using SPSS-11.5 and t-test. Results: Systolic and diastolic blood pressures and heart rate in 3rd minutes in interventional group following electroconvulsive therapy were 143.38±16 mmHg, 79.86±6.7 mmHg, 91.9±9.9 mmHg, respectively and in controls were 128.88±13.04 mmHg, 87.63±5.79 mmHg and 102.86±13 mmHg, respectively. These difference were significant (P<0.05). The above-mentioned indices for 5th minutes in intervention and controls were as follow: systolic (113.47±9.97 mmHg, 122.36±13 mmHg), diastolic (73.47±4.27 mmHg, 77.63±6.26 mmHg) heart rate (84.41±4.6 in minute, 93.19±12.53 in minute). These differences in above indices were significant (P<0.05). Conclusion: This study showed that lidocaine administration during electroconvulsive therapy increase the duration of convulsion and reduces heart rate and blood pressure.

Abstract

Background and Objective: The silent brain infarctions are common in first-ever stroke. Some recent studies show the possible role of silent brain infarction in ischemic stroke. The aim of this study was to evaluate the frequency of silent brain infarction in acute first-ever ischemic stroke and its association with stroke risk factors. Materials and Methods: This descriptive - analytical study was carried out on 203 (94 males, 109 females) patients with acute first-ever ischemic stroke in Shafa hospital in Kerman, Iran during 2010. The diagnosis of ischemic stroke was made by physical examination and was confirmed by MRI and CT Scan. A questionnaire containing demographic information and risk factors including hypertension, diabetes, hyperlipidemia, cigarette smoking, ischemic and non-ischemic heart disease for stroke as well as presence of silent stroke was completed for each patient. Data were analyzed using SPSS-16 and logistic regression test. Results: The mean age of patients was 62.56±17.35 years. Among our patients history of hypertension, hyperlipidemia, cigarette smoking, diabetes, history of ischemic heart disease, history of other cardiac disease and silent stroke were observed in 66 (32.5%), 26 (12.8%), 16 (7.9%), 40 (19.7%), 27 (13.3%), 16 (7.9%) and 31 (15.3%), respectively. The probability of silent stroke among patient with hyperlipidemia, was 3.7 times more than those without hyperlipidemia (95%CI: 1.556-12.780, P<0.05). There was no significant correlation between silent stroke and the above risk factors. Conclusion: Silent stroke was found in 15% of patients with ischemic stroke. Control of hyperlipidemia can have a vital role in the prevention of silent stroke.

Abstract

Background and Objective: Many studies have showed malformation of low frequency of electromagnetic fields (EMF) on different tissues. This study was carried out to evaluate the effect of low frequency electromagnetic fields on the heart of white-leghorn chicken embryo. Materials and Methods: In this experimental study, 90 healthy, fresh and fertilized eggs were allocated into 6 groups including control, sham, and four preincubated experimental groups. Experimental groups I, II, III and IV (1.33, 2.66, 5.52 and 7.32 mT) were located in the electromagnetic device, sham group was located into the same coil with no exposure for 24h before incubation. Control, sham and experimental groups incubated (37±0.5 ºC, 60% humidity) for 14 days. Results: Disassembling cell regulation in experimental group I, dense nucleus of myocytes and increase of intercellular spaces in experimental group II, necrosis and bleeding in the heart tissue in experimental groups III and IV were seen in compare to control and sham groups. There was a significant increase in the level of activity of alkaline phosphatase in the heart of experimental groups in compare to control and sham groups. Conclusion: Low frequency of electromagnetic fields caused alternations in cardiac tissue and elevation of Alkaline phosphatase activity in chicken embryos.

Abstract

Background and Objective: Free radical production andsubsqunt oxidative steress can be due to hyperglycemia and its oxidation. This study was done to evaluate the effect of swimming training test and Fenugreek seed extract on plasma glucose and antioxidant activity in heart tissue of streptozotocine – induced diabetic rats. Methods: In this experimental study, 50 male wistar rats were allocated into five groups diabetic (DC, n=10), healthy control (HC, n=10), swimming training (S, n=10), swimming training + Fenugreek seed extract (1.74 g/kg/bw) (SF1, n=10), and swimming training + Fenugreek seed extract (0.87 g/kg/bw) (SF2, n=10). Streptozotocine (60 mg/kg/bw) was used for induction of diabetes in DC, S, SF1 and SF2 groups. Serum glucose and the rat heart tissue antioxidant enzymes activities of superoxide dismutase, Catalase and Glutation peroxidase were determined. Results: Body weight in all groups were significantly reduced in comparsion with healthy control group (P<0.05). Plasma glucose level significantly reduced in SF1 and HC groups compared to diabetic group (P<0.05). Cardiac antioxidant enzymes in swimming training, SF1 and SF2 groups significantly increased in compare to diabetic group (P<0.05). Conclusion: The combination of endurance swimming training and fenugreek seed extract can reduce plasma glucose and increase cardiac antioxidant enzymes in streptozotocine – induced diabetic rats.

Abstract

Background and Objective: Imperforate anus is a common anorectal malformation, which is associated with other anomalies. This study was done to determine the associated malformations in newborns with imperforate anus in northern Iran. Methods: This descriptive study was carried out on 40 hospitalized newborns (24 boys and 16 girls) with imperforate anus in Taleghani teaching hospital in Gorgan, northern Iran during 2006-10. Results: From 40 newborns with imperforate anus, 23 newborns (57.5%) had associated anomalies. The commonest associated anomalies was genitourinary (65.2%) followed by heart (47.8%), gastrointestinal (13%) and musculoskeletal anomalies (8.7%). Conclusion: Associated anomalies in newborns with imperforate anus in northern Iran have a high prevalence in comparison with other reports.

Abstract

Background and Objective: Fetal heart pattern is one of the most important criteria in non stress test. This study was done to compare the effect of two anxiety- reducing methods, maternal relaxation and fetal-maternal attachment skills training on the prevalence of sporadic fetal heart decelerations and heart beat variability. Methods: In this clinical trial study, 126 pregnant women were randomly divided into three groups including teaching relaxation, maternal-fetal attachment skills and controls. Training was done for four weeks. NST was performed 30 minutes before of traning and after 4th week of training in the interventional groups. In the control group, NST was done in the 4th week. Cranley scale was filled at the end of the study. Results: The prevalence of sporadic deceleration in attachment skills, relaxation and control groups before intervention was 23.8%, 19% and 21.4%, respectively. After the intervention, the prevalence of sporadic deceleration in attachment skills group non-significantly changed to 11.9%. After the intervention the prevalence of sporadic deceleration in the relaxation group significantly changed to 4.8% (P<0.05). Conclusion: Maternal relaxation training decreased the prevalence of sporadic fetal heart decelerations, and enhances fetal heart beat variability.

Abstract

Background and Objective: A brief and short duration episode of ischemia is recorded in ischemic preconditioning (IPC). This latter condition provides a status in which large region of heart is protected when prolonged ischemia occurred. Levothyroxine play a protective role in IPC induction, and simultaneously with stress oxidative. This study was conducted to determine the protective effect of levothyroxine with oxidative stress reduction mechanism in ischemic preconditioning model in rat heart.
Methods: This experimental study was performed on 30 male Wistar rats in three groups of 10, as follows. In the reperfusion ischaemia group (IR), the heart of the animal was placed in a Langendorff apparatus. In the ischemic preconditioning group (IPC), prior to major ischemia, was exposed to 4 periods of 5-minute ischemia with reperfusion. In the intraperitoneally administered group, levothyroxine at a dose of 25 microgram per 100 g of body weight, the heart was exposed to reperfusion ischemia. The area of infarct and the level of malondialdehyde in the heart tissue were measured.
Results: The volume of Infarcted region in IR and IPC groups was 26.55 and 11.11 respectively. The same index for the Levothyroxine receiver was 12.56. Based on these findings it was demonstrated that Levothyroxine injection reduced the Infarcted region significantly similar with IPC (P<0.05). The MDA Levels in IR and IPC were 1328 and 777, respectively and in Levothyroxine group it was determined as 762. The size of Infarcted region in both IPC and treated with Levothyroxine groups significantly reduced in compared to IR group (P<0.05).
Conclusion: Injection of levothyroxine with ischemic preconditioning reduced the effect of reperfusion maladaptive ischemia in rat heart.

Abstract

Background and Objective: Dyspnea is the main cause of hospitalization of patients with heart failure and it is often associated with anxiety. This study was conducted to determine the effect of Chamomile tea on the severity of dyspnea and anxiety in patients with chronic heart failure.
Methods: This randomized clinical trial study was conducted on 60 patients with chronic heart failure in an outpatient clinic of Jahrom University of Medical Sciences, Iran during 2014. Patients were randomly divided into intervention and control groups. Subjects in intervention group consumed Chamomile tea for 4 weeks. Oxygen cost diagram (OCD) and Beck anxiety inventory were recorded for each subject.
Results: The mean of severity of dyspnea in the intervention and control groups was 16.66±5.76 and 29.43±13.1, respectively (P<0.05). The mean anxiety severity in the intervention and control groups was 12.07±1.1 and 6.7±6.2, 20, respectively (P<0.05).
Conclusion: Chamomile tea can reduces the severity of dyspnea and anxiety in patients with chronic heart failure.

Abstract

Background and Objective: Patients experience full stress condition after open heart surgery. Foot reflexology massage has a potential impact on the comfort of patients with special care. This study was done to evaluate the effect of soles of feet reflexology massage on feeling of comfort in open heart surgery patients.
Methods: This clinical trial study was done on 70 patients who were divided into intervention and control groups using permutable blocking method. 35 patients in interventional group were received Soles of feet Reflexology Massage after mesearment of vital sigins for six days. 35 patients in the control group did not receive any intervention received routine care. Data gathering tools were demographic items and HCQ questionnaire, which was completed in two stages, 24 hours after surgery, before massage and the 6th day after massage.
Results: The mean total comfort score in the intervention group was 103.1±9.04 and 110.63±7.6 before and after the intervention, respectively. This differnce was not significant. The mean total comfort score in control group was 105.14±10.37 and 111.29±7.56 perior and the end of study, respectively, this difernce was not significant. Also, there was no significant difference between considering the mean scors of comfort between the tow in the intervention and control groups.
Conclusion: Reflexotherapy massage had no effect on feeling of comfort in open heart surgery patients.

Abstract

Background and Objective: Gestational diabetes mellitus (GDM) is usually a disease caused by inadequate insulin production in pregnant women. GDM induces abnormal fetal growth.This study was done to evaluate the BMP2 and BMP4 genes expression in the development of the embryos heart in induced gestational diabetes of C57BL/6 mice.
Methods: In this experimental study, 8-week old adult C57BL/6 mice were randomly divided into diabetic and control groups. After mating of animals, the dams in diabetic group were received a single dose of 150 mg/kg/bw of streptozotocin on gestational day 1 of pregnancy, intrapereatonally. After 11.5 days of pregnancy, the embryos of both groups were extracted and heart tissue was extracted. RNA total tissue of the heart was extracted by trazole. After extracting RNA, expression of BMP2 and BMP4 genes in the heart of both groups was estimated by Real-time PCR.
Results: There was no singnificant diference in expression of BMP2 and BMP4 genes in the heart of 11.5 days of embryos in gestational diabetes mellitus group and control group.
Conclusion: Gestational diabetes mellitus had no effect on the expression of BMP2 and BMP4 genes in the development of the embryos heart.

Abstract

Background and Objective: Gestational diabetes (GDM) is a metabolic disorder which is caused by insufficient secretion of insulin. GDM is a risk factor for embryo during pregnancy and it is possible leads to congenital heart defects (CHD). Some of these defects may be due to a change in the expression of some of the important structural genes in the heart. Desmocollin 2 and collagen structural genes have important role in the cell adhesion of the cardiomyocytes.This study was done to determine the effect of gestational diabetes on expersion of desmocollin 2 and col5a2 structural genes in C57BL mouse embryo heart.
Methods: In this experimental study, 12 adult female and six adult male C57BL mice were used.After mating of the animals and observation of the vaginal plug, the female mice with vaginal plug were randomly divided into diabetic and control groups. At the first day of pregnancy, Induction of gestational diabetes mellitus in dams in the diabetic group was performed by the intraperitoneal (IP) injection of Streptozotocin with a dose of 150 mg / kg body weight per day in GD1. While in the control group, only citrate buffer was injected.Cesarean Surgery was done at E11.5 and embryo's heart was extracted from the body.Extraction of RNA, cDNA, and quantitative measurements of the amount of RNA were performed using Real -Time PCR.
Results: Induction of gestational diabetes increased the expersion of desmocollin 2 and col5a2 structural genes in compared to controls, althought only the expersion of desmocollin 2 gene was significant (P<0.05).
Conclusion: We suggest that the induction of DM lead to upregulation of structural genes primarily including desmocollin 2 and col5a2 in embryos heart development.
