Abstract

Cryptorchidism means undersecending testes, it is one of the most disorder in boy’s urogenital system. The aim of this research is to study the cryptorchidism prevalency in newborn boys in Gorgan. This research is a descriptive and cross-sectional study and the population under examination are those newborn boys that are given birth in Dizyani Hospital during 1377. 2318 newborn boys examined by standard physical method and the relation between this disorder and the following parameters has been the main concerns of this study, these are as follow: Either bilateral or unilateral cryptorchidism, mother’s age, parity, birth weight, maturating as birth, and race. The results of this study show, that from 2318 newborn boys 89 infant (3.8%) have cryptorchidism at birth. From those latter figures 2.3% are unilateral and 1.5% are bilateral. Our result indicate that there is a significant correlation between cryptorchidism gestational age (P<0.0001) and birth weight (P<0.0001) but there is no relation between cryptorchidism with mother age and parity. The present research also indicate that cryptorchidism prevalency in under-weight newborn and premature infant is considerably increased. The conclusion of our study indicate that cryptorchidism prevalency has got no significant differences from previous investigations.

Abstract

Low birth weight (LBW) is the main known determinant mortality in under 1 year old children particularly in neonates. The aim of this research is to study the rate of LBW and evaluation of some effective factors in LBW, such as: Difference races, residential conditions, maternal age, parity. This research is analytic-descriptive and cross-sectional study. The study population consists of 2183 newborns that are given birth in Dezyani Hospital during 1996. These samples were chosen by systemic randomized technic (3:1). Information was gathered by a questionnaire. The findings show that LBW rate was 6.3% in population under study, which in male and female newborns were 5.7% and 6.9% respectively. The LBW rates in newborns with mothers age?18 years old and those with ?35 years were 1.8% and 1.4% folds higher than LBW rates for newborns with mothers age 19-35 years. The LBW percentage for primiparous women were 1.9 folds to multiparous who had 2-4 deliveries. LBW rates for newborns with Systanie race were approximately 1.8 folds higher than newborns with Torkman ethnic. The results of this study indicate factors such as: Maternal age, race, parity, affect on LBW rates.

Abstract

Compared with older children and adults, neonates are more sensitive to pain and more vulnerable to long-term effects. Despite the clinical importance of neonatal pain, current medical practices continue to expose infants to repetitive, acute, or prolonged pains. This specific intervention are necessary for reduced the neonatal pain experiences. This is an experimental study of determine the effectiveness of skin contact between mother and her healthy full-term newborn to reduce pain experience by the infant during intramuscular injection. The sample population was 30 healthy full-term newborns delivered at university hospital in Gorgan. The samples and controls were chosen randomly. During intramuscular injection case group was being helped by their mothers under cloth that directly were contact with their abdominal and chest skin and newborn in control group swaddled in crib on the bed before, after and during injection. Behavioral responses (Facial changes, crying, …) and physiologic responses (Heart rate and O2 saturation of arterial blood) of newborns were noted before, after and during intramuscular injection. The results show that there was no significant difference in behavioral and physiologic responses caused by pain between two groups, except crying time (38.2 second in control group and 35.5 second in case group). Finding from this study indicate that skin-to-skin contact has a clinically important effect on the pain of intramuscular injection, if we can prepare physically and emotionally a good situation.

Abstract

Background & Objective: Respiratory distress syndrome (RDS) is one of the important causes of newborn mortality. The aim of this study is to determine the pregnancy outcome and Corticosteroid effects for prevention prophylaxis in pregnant women with premature labor between 34-37 weeks gestational age. Materials & Methods: This semi experimental single blind clinical trial study was performed on hundred 34-37 weeks pregnant women who were referred with labor pain or rupture of membrane. These women divided in 2 almost similar case and control groups. The control group received 5 mg Dexamethasone 6 hours to delivery time (Maximum 4 doses). Data was entered to SPSS and analyzed by Chi-square and T-test. Results: There wasn’t any significant difference between administration of Dexamethasone and incidence of RDS in 34-37 weeks newborn (P=0.6). The frequency of RDS was similar in 2 groups (In all women: 4%) the mean weight of newborns with RDS diagnosis was 2675±263. 76% of women had vaginal delivery. The mean weight of newborn was 2672 gr and mean gestational age was 35.4 weeks. Conclusion: The frequency of RDS in newborn with 34-37 weeks gestational age was 4% and administration of one or more Dexamethasone had no effect on reduction it.

Abstract

Background and Objective: One of the most important routes of hepatitis B virus (HBV) transmission is from asymptomatic carrier mothers to their infants. It has been concluded that over 50% of Iranian carriers have contracted the infection perinatally, making this the most likely route of transmission of HBV in our country. In this study, we assessed the rate of HBV infection in children were born to HBsAg positive mothers that received hepatitis B immunoglobulin (HBIG) and the first dose of vaccine at birth and the second and third of doses of vaccine at 1 and 6 months of age, respectively. Materials and Methods: In this descreptive study, 123 infants born from HBsAg positive mothers assessed and serum sample of these subjects were tested by ELISA technique for detection of HBsAg, Anti-HBs and Anti-HBc markers. Data were then analyzed by the chi-square and fisher exact tests. Results: From 31241 pregnant women with delivery during this 5-years study about 140 (0.45%) of them were HBsAg positive. Children born to these mothers follow-up and 123 serum samples of them tested for serologic markers of HBV infection. HBsAg was found in 0.8% of the children and 91.9% had one or more serologic markers of HBV infection. Protective rate (Anti-HBs positive) and Anti-HBc positive were 82.1% and 8.9% respectively. Conclusion: This study showed that after having focuced on the vertical transmission route for many years, and implementing strategies such as vaccination and HBIG injection to neonates of HBsAg positive mothers, nowadays it seems that we should pay attention to horizontal route of HBV transmission in this area.

Abstract

Background and Objective: Modification of life style, nutrional regiment and insulin therapy is used for improvement of pregnancy outcomes in pregnant women with gestational diabetes mellitus (GDM). This study was done to evaluate the effect of the dietary approaches to stop hypertension (DASH) eating plan on pregnancy outcomes in pregnant women with gestational diabetes mellitus. Methods: In this clinical trial study, 52 women with GDM were randomly divided into DASH and control groups for 4 weeks. The control diet contained 45-55% carbohydrates, 15-20% protein and 25-30% total fat. The DASH was rich in fruits, vegetables, whole grains and low-fat dairy products, and contained lower amounts of saturated fats, cholesterol and refined grains with a total of 2400 mg/day sodium. The numbers of women whom commenced insulin therapy after dietary intervention, the mode of delivery and prevalence of polyhydramnios were assessed. The length, weight and head circumference of infants were measured during the first 24 h after birth. Results: 46.2% of women in the DASH group needed to have a cesarean section, this rate for the control group was 80.8% (P<0.05). The percentage of those who needed to commence insulin therapy after intervention was 23% for DASH group vs 73% for controls (P<0.05). Infants born to mothers on the DASH group had significantly lower body weight (3222.7 vs 3818.8 g, P<0.05), head circumference (34.2 vs 35.1 cm, P<0.05) and ponderal index (2.50 vs 2.87 kg/m3, P<0.05) compared to those born to mothers on the control diet. Conclusion: Consumption of dietary approaches to stop hypertension for 4 weeks among pregnant women with GDM resulted in improved pregnancy outcomes.

Abstract

Background and Objective: Birth defects are important causes of childhood morbidity and disability. This study was done to determine the incidence and pattern of birth defects in live birth in cities of Golestan province, north of Iran. Methods: This descriptive study was carried out on 92420 live births in 13 hospitals in Golestan province, northern Iran from 21 January 2008 to 20 March 2011. The newborns were examined for the presence of birth defects. Gender, type of birth defects and residency of parents according to city in Golestan province was recorded for each newborn. Results: The incidence rate of birth defects, in Gorgan, Kordkoy, Aliabad and Gonbad was 20.46, 12.53, 10.86, and 8.99 per 1,000 live births, respectively. The incidence rate of birth defects western area (including Kordkoy, Bandargaz and Kordkoy), center (Gorgan, Capital city) and eastern area (including Aliabad, Gonbad, Minodasht and Kalaleh) of Golestan province) was 9.3, 20.46 and 8.79 per 1,000 live births, respectively. Cardiovascular anomaly was the most frequent birth defects. Conclusion: The incidence rate of birth defects varies in diferent area of Golestan provine and overally was lower than the other region in Iran.

Abstract

Background and Objective: Alpha Thalassemia is one of the most prevalent hemaglobinophaties worldwide. Alpha thalasseima patients may represent wide spectrum of symptoms ranging from asymptomatic to severe life threatening anemia. This study was done to assess the carrier frequency of alpha globin gene mutations among newborns in north of Iran. Methods: In this descriptive study, 412 cord blood samples of neonate from Amir Mazandari hospitali were randomly selected during 2012. Genomic DNA was extracted using phenol-chloroform method. Multiplex Gap- PCR and PCR-RFLP methods were applied in order to detect three common gene deletions, one triplication and one point mutation. Results: Total allelic frequency of investigated mutations was 0.0825. The -α3.7 deletion with allelic frequency of 0.0485 was the most prevalent mutation among 412 neonates. Allelic frequencies of -α4.2, αααanti3.7 triplication and α-5nt mutations were 0.0206, 0.0109 and 0.0024 respectively and -Med double gene deletion was not detected. Conclusion: Most mutated cases had single gene deletion that is asymptomatic while -Med double gene deletion was not detected among the neonates. Therefore, there is low probability of a child birth with Hb H disorder in the region.

Abstract

Background and Objective: Irrational consumption of blood products leads to unwanted side effects in patients and imposes additional financial burdens on society and patients. This study was done to determine the pattern of consumption of packed red blood cells (PRBC) in hospitalized newborns and children.
Methods: This prospective descriptive-analytic study was conducted on 80 newborn (less than 4 months age) 36 children (more than 4 months to 16 years old) who used PRBC in Sari Bu-Ali Sina hospital, north of Iran. Demographic characteristics of patients and parameters of blood product were recorded and compared with standard guideline.
Results: During the study period, administration of 116 units of PRBC (28.5%) was studied. Totally, in 24 % of cases PRBC consumption did not follow the guideline. PRBC consumption was inappropriately prescribed for patients with hemoglobin higher than 12 g/dL (12%) and for patients with hemoglobin higher than 10 g/dL (12%). Both of the consumption volume to weight ratio (P<0.05) and rate of infusion (P<0.05) were significantly higher in children less than or equal to 4 months, than in children older than 4 months. The most commonly indications were respiratory distress syndrome in neonates (62.1%) and bone marrow suppression (14.7%).
Conclusion: A significant percentage of patients were received PRBC out of guideline recommendations.  It seems, for rational use of PRBC in children and neonates the existing guidelines should be more considered to reduce the cost and side effects.

Abstract

Background and Objective: Perinatal mortality rate (PMR) is one of the main indicators in evaluation of health community and healthcare systems. This study was performed to determine the perinatal mortality rate in the referral third level of maternal and infant care center affiliated to Babol University of Medical Sciences.
Methods: This descriptive-analytical study was performed on by cross-sectional method and 19234 births (9751 male and 9483 female) from 22 weeks gestation to 29 days after delivery Ayatollah Rouhani Hospital in Babol, northern Iran during 2011-18. Data on the cause of neonatal death, maternal age, type of delivery, malformations, maternal underlying disease, maternal pregnancy-related conditions, and diseases at the time of death in neonate were collected from website of the Ministry of Health. Then each of the indicators was calculated per 1000 live births.
Results: In this study, the PMR was calculated per 1000 live births which was 29.01 (boys 16.17 and girls 12.84). The portion of vaginal delivery was 9.67 and cesarean section was 19.34. Out of 558 perinatal deaths, 59.9% died due to intrauterine death with an index of 17.37 and 40.1% of deaths were due to infant death with an index of 11.63. Most neonatal deaths (74%) occurred within the first 24 hours of birth and in terms of gestational age 86.02% were <37 weeks of gestation with an index of 24.95, and low birth weight were 83.8% with an index of 24.31. Underlying disease was found in 25.6% of mothers and pregnancy-related conditions in 74.4% of cases. Fetal distress in 14.4% with an index of 4.20 was the most common disease associated with infant death or cause of death in preterm infants. Other causes of perinatal death were specific disorders of perinatal origin, birth defects, chromosomal abnormalities, and accidents were 10.09, 4.06, and 0.58 per 1000 live births, respectively.
Conclusion: The most and important factors of prenatal mortality wrer prematurity and low birth weight that major causes include maternal pregnancy-related conditions and maternal underlying disease, Therefore, emphasis on adequate prenatal care, identification and follow-up of mothers with underlying diseases can be effective in reducing perinatal mortality.
