Abstract

Adrenocarcinoma is a rare disease with a poor prognosis. As a practical classification, adrenal carcinomas divide to two categories: functional and nonfunctional. Adjurent radiotherapy and chemotherapy is useful for palliation. The aim of this report is to introduce one case of large nonfunctional adrenocarcinoma. A 46 years old-male with ambiguous abdomen pain and orchidoepididymitis reffered to clinic in 2006. Physical examination revealed grade III fixed varicocele with left orchidoepididymitis. Sonography and computed tomography scan showed a massive lesion on left adrenal. Adrenal function test was normal, therefore nonfunctional adrenocarcinoma is diagnosed. This patient underwent open adrenalectomy. Then the patient introduced to chemoradiotherapy in oncology center. After six months, the patient suffered from multiple liver metastasis and one year later was expired.

Abstract

Background and Objective: Considering the significant incidence of nosocomial infections in hospitalized patients, this study was done to determine the prevalence of Staphylococcus aureus strains isolated from wound infection and drug sensitivity pattern, Tehran-Iran. Materials and Methods: In this descriptive study, Staphylococcus aureus isolated and identified according to standard procedures from the wound infections of 614 patients referred to Baqiyatallah hospital, Tehran-Iran during 2006-07. The samples were examined and antibiogram was performed by disc diffusion method on Mueller Hinton agar with 12 antibiotics. Results: 100 (16.28%) of wound infection of Staphylococcus aureus was isolated from 614 patients. The infection rate in men was twice compared to women. The highest rate 29 (29%) was observed in people aged 40 to 60 group. Also specimen's patients with immunosuppressive diseases (28 cases), surgical site infection (16 cases) and normal wounds (13 cases) were considered to be most prevalent isolates. Antibiotic sensitivity testing revealed that 96 (96%) of isolates were sensitive to vancomycin, 95 (95%) and 92 (92%) were resistant to penicillin and cotrimoxazole, respectively. Our result showed that 43% of strains were resistant at 11 antibiotics. Conclusion: This study showed that the prevalence of Staphylococcus aureus was 16.28% of samples, with 43% antibiotic resistance. The highest sensitivity was toward to vancomycin.

Abstract

Background and Objective: Irrational consumption of blood products leads to unwanted side effects in patients and imposes additional financial burdens on society and patients. This study was done to determine the pattern of consumption of packed red blood cells (PRBC) in hospitalized newborns and children.
Methods: This prospective descriptive-analytic study was conducted on 80 newborn (less than 4 months age) 36 children (more than 4 months to 16 years old) who used PRBC in Sari Bu-Ali Sina hospital, north of Iran. Demographic characteristics of patients and parameters of blood product were recorded and compared with standard guideline.
Results: During the study period, administration of 116 units of PRBC (28.5%) was studied. Totally, in 24 % of cases PRBC consumption did not follow the guideline. PRBC consumption was inappropriately prescribed for patients with hemoglobin higher than 12 g/dL (12%) and for patients with hemoglobin higher than 10 g/dL (12%). Both of the consumption volume to weight ratio (P<0.05) and rate of infusion (P<0.05) were significantly higher in children less than or equal to 4 months, than in children older than 4 months. The most commonly indications were respiratory distress syndrome in neonates (62.1%) and bone marrow suppression (14.7%).
Conclusion: A significant percentage of patients were received PRBC out of guideline recommendations.  It seems, for rational use of PRBC in children and neonates the existing guidelines should be more considered to reduce the cost and side effects.
