Abstract
This study was designed to evaluate the effects of total extract of Phytolacca Decandra on electrophysiological properties of atrioventricular node. Male rat heart after isolation was attached to a Langendorff perfusion system. The stimulation protocol was carried out during control condition (No intervention) and on the presence of selected concentration (1.10-7 %w/v, 2.10-7 %w/v, 5.10-7 %w/v) of Phytolacca Decandra. The basic (Wenchebach, AVCT and ERP) and functional (Recovery) properties of AV-node were studied in solated, perfused male rat cardiac preparations. Total extract of P.decandra significantly increased WBCL, AVCT and ERP in the concentration dependent manner, but had no significant effect on the time constant of recovery. The results of this study demonstrate that total extract of P.decandra had parasympathic-like effect on time-independent properties of AV-node and the above results also showed a potential anti-arrhythmic role of P.decandra in terminating supraventricular tachyarrhythmia.

Abstract
In this research we examined the sensitivity of micronucleus assay for monitoring clastogenic effects of low dose fast neutrons. Syrian mice (12 weeks old) were irradiated by fast neutrons emitted from a 241Am-9Be source. The absorbed dose was 1.5, 2.25, 3.375 and 5.06 cGy. Mice were scarified by cervical dislocation at different post irradiation times (24, 48 and 72 h). The results obtained show that the frequency of neutron-induced micronuclei in polychromatic erythrocytes (PCES) is significantly higher than those of control groups (P<0.05) at neutron dose used in this experiment. We concluded that micronucleus assay is an effective and also inexpensive method for monitoring clastogenic effects of high LET radiation in low dose levels.

Abstract
Cryptorchidism means undersecending testes, it is one of the most disorder in boy’s urogenital system. The aim of this research is to study the cryptorchidism prevalency in newborn boys in Gorgan. This research is a descriptive and cross-sectional study and the population under examination are those newborn boys that are given birth in Dizyani Hospital during 1377. 2318 newborn boys examined by standard physical method and the relation between this disorder and the following parameters has been the main concerns of this study, these are as follow: Either bilateral or unilateral cryptorchidism, mother’s age, parity, birth weight, maturating as birth, and race. The results of this study show, that from 2318 newborn boys 89 infant (3.8%) have cryptorchidism at birth. From those latter figures 2.3% are unilateral and 1.5% are bilateral. Our result indicate that there is a significant correlation between cryptorchidism gestational age (P<0.0001) and birth weight (P<0.0001) but there is no relation between cryptorchidism with mother age and parity. The present research also indicate that cryptorchidism prevalency in under-weight newborn and premature infant is considerably increased. The conclusion of our study indicate that cryptorchidism prevalency has got no significant differences from previous investigations.

Abstract
The treatment of supraventricular arrhythmia include wide range of medical intervention, but ideal drug for treatment of this kind of arrhythmia is yet to be developed. Pharmacological interventions due to their adverse side effects and the possibility of proarrhythmic effects are usually ineffective in treating these conditions. Herbal remedies are suitable alternatives for synthetic drugs, due to their availability, minimal side effects and lower price. Pharmacological studies and traditional medical literature point to the cardiovascular effects of the Achillea Santolina in many instances. In present study, we used isolated heart of langandrof rats as an experimental model to determine the effect of various concentrations of the methanol extract of Achillea santolina (2×10^-7, 2×10^-9, 2×10^-10 W/V) on the electrophysiological properties of the isolated heart. Results of this study show a significant depression of WBCL, AVCT and ERP and non-significant increased in time constant of recovery (trec). It may be therefore be considered a potential role for anti-arrhythmic effect of Achillea Santolina in suppression or treating supraventricular tachyarrhythmia.

Abstract
Breast cancer is the most common cancer in women. Histologic grading is one of the prognostic factors in breast cancer. The present study performed in analytic descriptive method and based on the slide review of beast biopsies received in the pathology department of 5th Azar Hospital from 1976 to 1978, and on the basis of Bloom-Richardson criteria (Mitoses, tubule formation and nuclear pleomorphism). The results are consist of: 1) Infiltrating duct carcinoma is the most common histopathologic form, and tubular carcinoma is the least common 2) The most common age is 36-45 years 3) There is meaningful relationship between mitoses and tubule formation and between pleomorphism and tubule formation. But there is no relation between nuclear pleomorphism and tubule formation in the tumor. Histologic grading of breast carcinoma should be reported by pathologist for clear determination of prognosis and also the best choice for management of the tumors.

Abstract
Low birth weight (LBW) is the main known determinant mortality in under 1 year old children particularly in neonates. The aim of this research is to study the rate of LBW and evaluation of some effective factors in LBW, such as: Difference races, residential conditions, maternal age, parity. This research is analytic-descriptive and cross-sectional study. The study population consists of 2183 newborns that are given birth in Dezyani Hospital during 1996. These samples were chosen by systemic randomized technic (3:1). Information was gathered by a questionnaire. The findings show that LBW rate was 6.3% in population under study, which in male and female newborns were 5.7% and 6.9% respectively. The LBW rates in newborns with mothers age?18 years old and those with ?35 years were 1.8% and 1.4% folds higher than LBW rates for newborns with mothers age 19-35 years. The LBW percentage for primiparous women were 1.9 folds to multiparous who had 2-4 deliveries. LBW rates for newborns with Systanie race were approximately 1.8 folds higher than newborns with Torkman ethnic. The results of this study indicate factors such as: Maternal age, race, parity, affect on LBW rates.

Abstract
Amniotic band syndrome is a collection of congenital abnormalities caused by early amnion rupture. It occurs in approximately 1:1200 to 1:15000 live births. The infant we are reporting was as result of dizygotic twins pregnancy of 34-years old woman in first delivery than born by cesarean section. In clinical examination there were multiple defects such as: Anencephaly, anophthalmia, cleft lip-palate, nose malformation and limb anomalies. Involved infant died an hour after birth, because of severity of anomalies. This case was reported because of rarity and severity signs of this syndrome and its occurrence in one twin from dizygotic twins.

Abstract
Rubella can be a disastrous disease in early gestation, leading to fetal death, premature delivery, and an array of congenital defects. In this study we tried to determine the preset states of immunity against Rubella virus among high-school girl pupils in Gorgan city in the north east of Iran. A total of 484 blood samples collected randomly from 484 school girl pupils for the detection of antibody against Rubella virus. Rubella antibody determined by hemagglutination inhibition (HAI) test. From total of 484 high-school girl pupils 88 (18.2%) were found susceptible to Rubella infection and 396 (81.8%) were immune to this infection. Our results show significant relation between immunity, age and history of previous vaccination against Rubella.

Abstract
In a randomized study we compared postoperative pain and analgesic requirement in patients who underwent elective cesarean section under general anesthesia induced with Thiopental 4 mg/kg (N=25) or Ketamin 1 mg/kg (N=25). Anesthesia was maintained with N2O and Halothane. Postoperative pain was measured with OPS scale and analgesia was provided with Morphine. Median time to first need of analgesic was greater in Ketamine group compared with Thiopental group. Median Morphine consumption over 24 hour was less in Ketamine group compared with Thiopental group. No patient had recall of intraoperative events in Ketamine group however in Thiopental group 12% of patients had such recalls. Apgar score was similar between groups. Induction of anesthesia for cesarean section using Ketamine is associated with a lower postoperative analgesic requirement compared with Thiopental. Patients who had anesthesia for cesarean section induced with Ketamine required less analgesic drugs in the first 24 hours postoperative period compared with patients who received Thiopental. Ketamin unlike Thiopental has analgesic properties that may reduce sensitization of pain pathways and extend into post-operative period.

Abstract
Golden Staphylococci is one of the most common factors in skin and systemic infection. About 30% of people are carrying this bacterium in their skin and nose. Increased drug resistance in this microorganism and consequent infection, has gained the attention of scientific organization. Therefore we decided to determine the prevalence of nose carrier of this bacterium in healthy children in Gorgan schools. During the winter 1998 to March 1999 the sample has been taken from 1193 student’s nose with sterile cotton swab. These results showed that in 194 samples (16.3%) Staphylococci Aureus has been isolated, from these samples 114 cases belong to girls (16.1%) and 80 cases were boys (16.4%) that there is not a meaningful variation between them. The number of bacteria isolated from the villages children was 109 cases (17.6%) and in town’s children were 85 cases (14.8%), also this difference didn’t show significant difference. The prevalence of carrier among the 6-12 years old was 12-25.3%, which has got a significant meaning (P<0.001). Antibiotic resistance in the isolated S.aureus were tested and result showed that 34.8% of samples are resistant to Methicilin and 1.7% were resistant to Vancomycin, only 4% were sensitive to Penicillin. Therefore with regard to the results of this investigation, further researches are recommended in relation between strain’s of S.aureus that isolated from carrier’s and strain’s that isolated from patients and to determine their drug resistance in this region.

Abstract
Documentation is one of the most important nursing functions that one third of their time are spent for them. Since documentation is recording all of cares delivered to patient, the result of the care and patient response to treatment, and all that has been done does show in documentation. The present research is the semi-experimental study with purpose of determining effect of continuing education on quality of nursing records. The samples consist of nursing personnels of medical-surgical units that responsible for documenting of nursing records Gorgan teaching hospital. Prior to and after education, the content and structure of records were evaluated, scored and mean of scores were comparated by a check-list analysis of findings indicated significant differences between mean of scores prior to and after education (P<0.001). The results showed that we can use continual education for promotion of nursing record quality.

Abstract
Pregnancy is the great physiological stress during of women’s life and cause the most importance change in their biological state. This physiological stress arising from fetus needs and thus increase pregnant women requirement to energy and nutrients in comparison with non-pregnant women. Nutritional continuous training caused increase of women knowledge by additional foods side, for poor mother, provide maternal and child health, so this survey has been done to determine knowledge rate, attitude and practices of pregnant women in Tehran, nutritional knowledge, attitude and practices in 315 pregnant women were assessed in 10 hospitals of Tehran. A structured questionnaire was used for this purpose. This research showed, 60% of women had low-level nutritional knowledge. In the attitude part of this research data showed 71.4% of women had positive attitude toward changes to improve the quality of their diet, and 87.6% of women had positive attitude toward dairy consumption eating during pregnancy. But 69.8% of women had negative attitude about Iron supplement. There was significant relation between level of education and attitude toward Iron supplement. Nutritional knowledge of pregnant women is weak so that the effects in their practice. Therefore nutritional training is necessary during pregnancy by physician.

Abstract
Gastro-enteritis due to Vibrio Cholera was increased suddenly in summer of 1998 in our country. In this research we studied epidemiological and clinical features of Cholerae patients. Vibrio Cholera isolated from 189 (3.3%) patients from 56II stool samples. 182 (96.2%) Vibrio Cholera O1 isolated. 46 (24%) patients with positive stool cultures for Vibrio Cholera were hospitalized. 50% patients were 15-45 yrs. No sex predominency was seen. The important observations in this study including: Fever in 28.1% (13) of patients, sever leukocytosis in 15% (7) of patients, RBC in stool in 33% (15) of patients. Fever and neutrophilia and RBC in stool probably shows co-infection with order intestinal pathogens or different serotypes of Vibrio Cholera with new characteristics. So we are recommending extensive research to be done on serotyping of all of the isolates of Vibrio cholera and other intestinal pathogens simultaneously.

Abstract
Compared with older children and adults, neonates are more sensitive to pain and more vulnerable to long-term effects. Despite the clinical importance of neonatal pain, current medical practices continue to expose infants to repetitive, acute, or prolonged pains. This specific intervention are necessary for reduced the neonatal pain experiences. This is an experimental study of determine the effectiveness of skin contact between mother and her healthy full-term newborn to reduce pain experience by the infant during intramuscular injection. The sample population was 30 healthy full-term newborns delivered at university hospital in Gorgan. The samples and controls were chosen randomly. During intramuscular injection case group was being helped by their mothers under cloth that directly were contact with their abdominal and chest skin and newborn in control group swaddled in crib on the bed before, after and during injection. Behavioral responses (Facial changes, crying, …) and physiologic responses (Heart rate and O2 saturation of arterial blood) of newborns were noted before, after and during intramuscular injection. The results show that there was no significant difference in behavioral and physiologic responses caused by pain between two groups, except crying time (38.2 second in control group and 35.5 second in case group). Finding from this study indicate that skin-to-skin contact has a clinically important effect on the pain of intramuscular injection, if we can prepare physically and emotionally a good situation.

Abstract
Toxoplasmosis is a disease caused by a ubiquitous, obligate intra cellular protozoan Toxoplasma Gondii. Based on serological evidences, Toxoplasmosis infection is a widespread disease with regard to this fact and because to the occurrence of congenital Toxoplasmosis infections due to asymptomatic infections in pregnant women, determination of the rate of Toxoplasma antibodies (IgG, IgM) before pregnancy and finding the involved factors in increasing Toxoplasmosis prevalence seems to be necessary. This study was a cross-sectional and descriptive study and sample population was the women referred to marriage consultation the prevalence of Toxoplasma antibodies (IgG, IgM) was determined by ELISA method. Blood sample were collected randomly from 300 women referred to the center for consultation in 2001 and they was transferred to the laboratory for antibodies determination. The results showed that the general prevalence to the positive cases based on high titer of IgG was 48.3% and of IgM was 11.7%. There was not any relation between positive cases and age, education, place for residence, job, keeping domestic animals (Except cat) vegetable consumption and wash-up, but there was a relation between positive cases of IgM and keeping cat at home (P?0.025). More over 51.7% of pregnant women in Gorgan were seronegative and the were prone to acute Toxoplasmosis during their pregnancy. The results of this study confirm that the determination of the diagnostic Toxoplasmosis is an necessary test during pregnancy.

Abstract
All the different kind of Mycobacterium species except Tuberculosis Leprae and Bovis are considered as environmental Mycobacterium, which usually can be isolated from sand, water and dusts. In addition to the role of their pathogenesis, they can stimulate the immune systems. The immune mechanisms following Mycobacterium environmental contamination can be either protect or destroy the immune system when facing the disease related to the Mycobacterium. The Golestan province has got 2 regions from the point of view of TB epidemy, it is higher in the east. The aim of this project is determine the prevalency of various types and strains of environmental Mycobacterium in the east and west of the region. The samples were collected from both regions from the wet sand, and muddes covered with grass and following their preparation they were cultured on the Lowen Stein Jenson. The total of samples were 220 from these 120 samples were from the region with epidemy of TB out of these we had 25 samples with positive culture (20.8%) and 47 type of mycobacterium were isolated, the most common forms of them were Mycobacterium Fortuitum (34%), Mycobacterium Flavesens (21.2%), Mycobacterium Chelonae (12.8%). From 100 samples taken from the region with low epidemy of TB 66 samples were with positive culture (66%) which 114 strains of Mycobacterium were diagnosed. The most common forms of these microorganisms were Mycobacterium Flavesens (20.1%), Mycobacterium Chelonae (18.4%) and Mycobacterium (16.6%). In conclusion in all the Golestan province without taking notice the TB epidemy form 220 samples 91% were positive culture (41.2%) which 161 different strains were diagnosed out of these the most common were Mycobacterium Fortuitum (21.8%), Mycobacterium Flavesens (20.5%) and Mycobacterium Chelonae (16.8%). The epidemy and variousity of environmental Mycobacterium in the region with low epidemy were much higher than the region with high epidemy and in regard to that these differences are due to the bioenvironmental factors. So therefore this can indicate that the possibility of higher contact between the immune systems and the environmental Mycobacterium. These Mycobacteriums can act as Booster of BCG vaccine which can continuously stimulate the immune systems and this can help the body’s proper responses when facing with the TB Mycobacterium.
