Abstract

Background and Objective: Nowaday many diabetic patients interested in using medicinal herbs to relieve the symptoms of their disease in spite of the availability of synthetic drugs, one of such herbal medicine is green tea, studies about effects of this plant on blood glucose, weight of diabetes is contradict and suitable dosage is not mentioned, thus this study was done to determine the effect of green tea extract on blood glucose and body weight in male induced diabetic Rats. Materials and Methods: In this experimental study 40 wistar male Rats with body weight of 230±20 purchased and kept standard situation subsequently 8 of them considered as normal group and the reminder became diabetic by 50mg/kg IP by streptozotosin. After 2 weeks animals with blood glucose of 200-500 mg/dl were considered as diabetic and divided in 4 groups with 8 animals in each. Then 3 groups of them daily was feed with 50, 100 and 200 mg/kg of extract by feeding tube for 6 weeks, diabetic control group and non-diabetic group also was feed with DMSO10% as well. BS and body weight were assessed at 2nd, 4th, 6th week after daily feeding by different doses of green tea extract, and at last results were analyzed by using student t-test and one way ANOVA. Results: The mean of blood glucose of diabetic control group were significantly higher than non-diabetic group and blood glucose of diabetic groups that was feed with 50, 100 and 200mg/kg of extract was significantly lower than diabetic control group. Body weight of 3 treatment groups were not significantly reduced. Conclusion: This study showed that green tea extract has an antidiabetic effect and suitable dosage of this extract was 100mg/kg.

Abstract

Background and Objective: Although directly observed treatment (DOT) strategy had decreased tuberculosis (TB) mortality, but in developing countries, TB is yet one of the leading cause of deaths. Inappropriate treatment regimens and poorly planned and one of the TB control programs have contributed to rise in its incidence. The purpose of this study was to describe knowledge, attitude and compliance with tuberculosis treatment. Materials and Methods: This sescriptive – analytic study was done on 167 tuberculosis patients in Ahvaz-Iran during 2009. A questionnaire, observation, sputum smear, potts – cozart test (for detect of isoniazide metabolites in urine) was used for data collection. Reability of questionarie was determined by alpha-cronbach correlation method (α=0.81). Data analyzed with Chi-Square test. Results: The findings showed that 52.7%, 35.3% and 12% of the patients demonstrated good, midscores and poor adherence to drug regiment respectively. Also there was a significant relation beween control of medication and knowledge and attitude in tuberculosis patients (P<0.05). Conclusion: This study showed that a half of patients used medication according to DOT strategy, knowledge and attitude were effective on quality of adherence to the medication.

Abstract

Background and Objective: Production of beta-lactamase enzymes is the most common mechanism of bacterial resistance against beta-lactam antibiotics. The aim of this study was to determine the antibiotic resistance pattern and the frequency of AmpC genes in Escherichia coli (E.coli) isolated in outpatients.
Methods: In this descriptive-analytical study, 67 isolates of E.coli were investigated from urinary tract infection of outpatients of the largest medical center in Kermanshah, west of Iran. Their susceptibility to ceftriaxone, cefotaxime, ceftazidime, cefoxitin and imipenem antibiotics was determined using disk diffusion. AmpC phenotypic screening was performed using combination disk method (cefoxitin with and without boronic acid). After extraction the bacterial genome, the presence of MOX, CIT, DHA, ACC, EBC and FOX genes were tested by multiplex PCR.
Results: The resistance of 67 E.coli isolated to ceftriaxone, cefotaxime, ceftazidime and cefoxitin was 49.2%, 49.2%, 37.3% and 25.3%, respectively. The 100% of the isolates were sensitive to imipenem. Seventeen (25.3%) and 9 isolates (13.4%) were phenotypically and genotypically positive for AmpC, respectively. The prevalence of CIT, MOX, FOX, DHA and EBC genes was 7.4%, 5.9%, 4.4%, 4.4% and 2.9%, respectively. However, the ACC gene was not found in isolates. Except for significant correlation between AmpC phenotype and MOX gene (P<0.05), no significant statistical relationship was found between phenotype and AmpC genotype. There was a significant correlation between AmpC phenotype and ceftazidime antibiotic (P<0.05). There was a significant correlation between CIT gene and EBC and FOX (P<0.05).
Conclusion: AmpC-producing E.coli isolates cause significant resistance to cephalosporins. One of the current therapeutic options is using of carbapenems. However, the relatively high prevalence and synergistic genes of AmpC in outpatients are a big concern and unfortunately it reflects the fact that these isolates are prevalent in the society.

Abstract

Background and Objective: Thyroid masses are one of the most common reasons for patients to visit endocrinology clinics. Fine needle aspiration (FNA) is used to reduce thyroid surgery for benign lesions. This study was done to determine the diagnostic accuracy of FNA of thyroid mass(s) compared with histological results in Medical Centers, Gorgan, Iran.
Methods: In this descriptive-analytical study, 102 patients (12 males and 90 females) were diagnosed with a thyroid mass and referred for surgery to Medical Centers, Gorgan, Iran during 2020-21. FNA was performed, and the samples were sent to the pathology department for examination. The result of cytology was recorded in the checklist. According to cytology results, some patients were referred for surgery. Then, the surgical pathology results were compared with the cytology findings. In addition, the sensitivity and specificity of the results of the cytology of thyroid masses were calculated compared to the results of pathology.
Results: FNA, mass size, and the patient’s main complaint had a significant relationship with histological results (P<0.05). According to FNA findings compared to pathology, sensitivity was 74%, specificity was 88.1%, positive predictive value was 88.1%, negative predictive value was 74%, false positive rate was 12%, false negative rate was 26%, and accuracy was 80.4%.
Conclusion: FNA remains a key tool in evaluating thyroid nodule(s). Improving FNA techniques is necessary to increase the accuracy of FNA.
