Abstract

Background & Objective: Destructive effects of ethanol consumption have been confirmed on several organs of the body. Nevertheless, classic research which has been done on kidney in this field had less attention. The aim of this study was to evaluate of ethanol effects on light microscopic structure of Mice kidneys. Materials & Methods: In this experimental study, Balb/c strain mice, equally 40 males and females, weighing 30-35 grams were divided in two equal experimental and control groups randomly. The Mice in control and expermented groups further divided in two sub groups. One sub group received 1mg/gr body weight daily of ethanol for one month, intraperitoneally and other sub group received 1mg/gr body weight of ethanol for two months, intraperitoneally. Control groups divided in two sub groups and Mice received daily salin with same volume as experimental groups. Then, the Mice have been anesthetized and sacrificed and then, subsequently kidneys were removed. Routine histological methods and Hematoxylin and Eosin staining were done. Slides were observed by light microscope. Results: In experimental group, Mice which received ethanol for one month, interstitial bleeding and inflammatory cells infiltration in cortical zone of kidney was observed. In Mice which received ethanol for two months, we observed massive and concentrated lymphoblastic infiltration, especially on deep medulla around the blood vessels with extending to calyxes and pelvis of the kidney. No histological changes were observed in control groups. Conclusion: This study showed that the intraperitoneal injection of ethanol cause progressive destruction effects on cortex and medulla of the kidney. These effects depend on duration of ethanol consumption.

Abstract

Background and Objective: Anterior cruciate ligament (ACL) tear is one of the most common knee injuries among young adults and sportman. They need reconstruction when they are accompanying with other knee internal derangements to prevent knee osteoarthritis. This study was designed to explore early complication of anterior cruciat ligament reconstruction with bone-patellar tendon- bone graft. Materials and Methods: This prospective and cross-sectional study was done on 13 patients which refered to Qaem hospital in Mashhad-Iran during 2008. The first 3 months complications and problems of anterior cruciate ligament reconstructions, with bone-patellar tendon, bone graft technique, was recorded on regular basis, symptoms and signs were documented. Results: Anterior knee pain and knee stiffness were common as 70% and 38% respectively. We did not observe any infection but in 15% of patients a minor instability (ADT+) was detected. Patients cooperation was low and even 54% of them did not implement, post-operative advices. Conclusion: This study showed that minor instability and subsequent pain are the early problems during the first three month after anterior cruciate ligament constraction.

Abstract

Background and Objective: There are some reports about probable association between acute leukemia and special blood groups. Frequency of ABO and RH blood group is different in each population. This study was done to determine frequency of ABO and Rh blood groups in patient with acute leukemia and healthy population.

Materials and Methods: This case - control study was done on 214 patients (132 males, 82 females) that suffering from acute leukemia as case group and 117026 individuals healthy population who were voluntary blood donors as controls in Mashhad, Iran during 2001-07. Age, sex, ABO blood group and RH blood group were collected from hospital files for each cases. Furthermore the results of ABO and RH blood groups of controls were collected. Data were analyzed with Chi-Square, fisher test and Odd’s ratio.

Results: The frequency of A, B, AB, O and RH blood groups in cases were 23.8%, 32.8%, 7.0%, 36.4% and 90.7%, respectively. The frequency of A, B, AB, O and RH blood groups in controls were 29.8%, 27.4%, 8.9%, 33.9% and 88.3%, respectively. Odds Ratio test didn’t show association of ABO and RH blood groups with acute leukemia. Odds Ratio test showed association of B blood group with acute leukemia in females (OR=0.571, 95%CI: 0.358-.0908, P=0.021).

Conclusion: This study showed although there was not any association between ABO and RH blood groups and acute leukemia but probably females with B blood group are more susciptible toward acute lymphoblastic leukemia.

Abstract

Background and Objective: Child obesity is increased in developing countries. This study assessed the prevalence of obesity on children which are exclusively on breast feeding and its duration among children aged 24-59 months in Bandar Turkmen district Northern, Iran.

Materials and Methods: In this descriptive- analytical study, 343 in children aged 24-59 months were selected with stratified random sampling. Obesity was defined as >+2SD NCHS (National Center for Health Surveys) reference. Data were collected by questionnaire, interview with mothers and were analyzed with Chi-Square and Fisher tests and multiple regression logistic models.

Results: In this study, 6.4% of children were obese. Prevalence of obesity was not significantly different between boys and girls (5.4% and 7.4% respectively). Breast feeding duration<18 months (P<0.005) and formula or cow’s milk plus breast feeding up to 6 months old (P<0.01) was significantly correlated with children’s obesity. Breast feeding duration more than 18 months reduced the obesity risk factor with 64%.

Conclusion: This study demonestrated that exclusively breast feeding in the first 6 months of life and breast feeding duration more than 18 months reduce the child obesity.

Abstract

Background and Objective: Legg-Calve-Perthes disease is one of the most common disabling diseases of childhood and if not managed appropriately is truly disabling. Severity of disease and duration of disease onset are main determinants of choosing treatment strategy of either conservative management or surgical treatment. This study was designed to determine Legg-Calve-Perthes treatments and relation between type of treatment and duration of the disease.

Materials and Methods: This descriptive study was carreid out on 50 patients with Legg-Calve-Perthes disease admitted in Ghaem Hospital in Mashhad, North East of Iran from 1995-2005.

Results: According to age of patients and severity of disease, the subjects allocated to conservatine (11 patients) and surgical treatment (39 patients). Clinical improvment signs include pain reduction, range of joints motion, which were avaluate by physical examination and taking medical history of patients. In conservative group, only 4 patients assumed to be successfully treated with mean age of 7.25 years and the mean interval between disease onset and beginning of treatment was 2.25 months. Surgical group had mean age of 10.85 years and mean interval between onset of disease and initiation of treatment was 18 months. Conservative group showed less sever form of disease (lateral pillar group A and group B with age less than 8 years), but patients treated surgically were more severely involved (lateral pillar group B with age more than 8 years and group C).

Conclusion: This study showed that age and severity of disease (lateral pillar classification) are main elements in treatment strategy (choosing conservative VS surgical treatment). The interval between onset of disease and onset of treatment strongly affects treatment success.

Abstract

Background and Objective: The neurohypophysis originates from the floor of diencephalon. Its development controles by several cellular interactions that mediated by some molecules such as cell surface and extra cellular matrix Glycoconjugates terminal sugars. In this study we used lectin histochemichal technique to evaluate distribution of the Glycoconjugates and their changes during development of neurohypophysis. Materials and Methods: This experimental study carried on 40 female and 20 male adult Rats. After mating and appointment day zero of pregnancy, pregnant Rats were sacrificed from days 10-20 of gestation and their embryos were collected for histochemical study. The serial section of head specimens were fixed and incubated with different HRP-lectins from Orange fungus (OFA) Vicica villosa (VVA), Glycine max (SBA), Wistaria floribunda (WFA), peanut (PNA), Griffonia simplicfolia (GSA1-B4), Lotus tetragonolobus (LTA) and Ulex Europeus (UEA-1). OFA, LTA and UEA-1 lectins are specific for terminal sugars α-L–Fucose and WFA, SBA, VVA and PNA are specific for D-GalNAc, α, ß-D-GalNAc and GalNAc, D-Gal-(ß-1-3)- D-GalNAc of complex glycoconjugates respectively. Results: Our findings demonstrated that the reaction of neurohypophysis cells with OFA initiated from gestational GD10 and increased to GD15 (P<0.05) and then increased to GD17 (P<0.05). A few cells of neurohypophysis reacted with PNA from GD13 to GD16 and decreased afterward (P<0.05). Some cells of neurohypophysis reacted with SBA from GD14 to GD18 and decreased afterward (P<0.05). Reacting of many cells of neurohypophysis with WFA started on GD13 and increased to GD15 (P<0.05) and then decreased afterward (P<0.05). Neurohypophysis cells showed no reaction with the UEA-1, LTA, VVA and GSA1-B4 lectins. Conclusion: The expression of Glycoconjugates with terminal sugars α-L–Fucose, α, ß-D-GalNAc and D- Gal– (ß-1-3)- D-GalNAc have importanct role and special spatiotemporal situation in neurohypophysis development.

Abstract

Background and Objective: The Detection of neurologic signs for topographical disorders in central nerves system can prevent unnecessary neuroimaging techniques such as MRI. This study was performed to determine the diagnostic value of physical examination for topographic detection of infratentorial lesions in patients with cerebrovascular syndromes. Methods: This descriptive study was done on 200 patients with cerebrovascular syndrome in Qaem Hospital in Mashhad, north-east Iran during 2011. Regarding to topographic physical examination, sensitivity, specificity, positive and negative predictive value and accuracy of signs of syndromes were compared to MRI as gold standard method. Results: The accuracy of tetraparesis and crossed syndromes for localization of brain lesion in brainstem was 79% and 83%, respectively. The accuracy of hemiataxia for localization of brain lesion in ipsilateral cerebellar hemisphere was 98%. The accuracy of Wallenberg syndrome for localization of brain lesion in posterolateral medulla was 98%. Conclusion: Using clinical neuro-examination skills and accurate topographic physical examination can prevent unwanted MRI technique for the diagnoses of cerebrovascular syndromes.

Abstract

Background and Objective: Fungal ear infection is common diseases in tropical areas with manifestation of acute and chronic clinical symptoms in external ear canal. This study was done to compare the fungal flora in external ear canal in chronic otitis media and subjects without otitis media. Method: This case-control study was done on 63 patients with chronic otitis media and 63 subjects without otitis media. Samples were taken from external ear canal with a sterile swab and were placed in the sterile tubes containing normal saline. The direct smear was prepared and samples were cultured in S, SCC and CMA (corn meal agar) media. Identification of genus and species were established using slide culture method and Germ-tube assay. Results: Positive fungi culture in external ear canal was seen in 77.78% and 17.46% of case and control groups, respectively (P<0.05). The most common type of fungi was Saprophyts (57.33%) followed by yeast (20.59%) and dermatophytes (17.32%). The most prevalent fungi in the subgroup of Saprophyts and dermatophytes was Aspergillusnigra (41.66%) and Trichophytonmentagrophytes (36.37%), respectively. The most common subgroup in yeast was Candida SPP with 53.85%. Conclusion: This study showed that the positive fungi culture in external ear canal is more prevalent in patients with chronic otitis media.

Abstract

Background and Objective: Different organizers are involved in spinal cord development and differentiation by sending various messages. Specific glycoconjugates secreted from the cells of lateral wall of spinal cord can also act as neurogenesis and neural differentiation messengers. This study was carried out to determine the distribution of sugar compounds in the lateral walls of spinal cord during mice morphogenesis using lectin histochemistry method. Methods: In this experimental study, sections of BALB/c mice from 10-16 embryonic days were fixed in formalin and then histological sections were prepared. Tissue samples for reaction to the glycoconjugates were incubated with DBA, OFA, GSA1B4 and MPA lectins. Alcian blue with pH equal 2.5 was used for background staining. Results: DBA lectin did not react with the lateral wall of the spinal cord. MPA lectin showed severe reaction but consistent, especially in nerve fibers of the lateral wall of spinal cord. GSA1B4 lectin showed weak reaction in the cells and nerve fibers of the spinal cord, but severe reaction was clearly observed in blood vessels. OFA lectin showed severe reaction with α-L-Fucose terminal sugar in the lateral walls of the spinal cord in early stages of morphogenesis. Conclusion: The most reaction in the lateral walls of the spinal cord was related to OFA, which reflects the importance of fucose terminal sugar by connecting (1→6) to the penultimate sugar N-acetyl-D-glocosamin (Glc-Nac) in the development of spinal cord. Due to severe reaction of GSA1B4 to blood vessels of spinal cord, use of this lectin for vascular studies, is recommended.

Abstract

Background and Objective: The anti inflammatory, analgesic, antioxidant of Sambucus ebulus L. have been reported in several studies. This study was done to assess the repelling property of the methalonic and hexanic extracts of Sambucus ebulus L. against the Culex pipiens.

Methods: In this experimental study, Sambucus ebulus L. collected from the natural inhabitants of Mazandran province in northern Iran. Methalonic and hexanic extraction were provided from the leaf and fruit of Sambucus ebulus L. Concentration of 50 mg/kg, 100 mg/kg and 250 mg/kg was prepared. 0.4 ml of the extract prepared and was spreed on the albino skin area of 4×6 cm2. After 30 minutes the number of the mosquito (Culex pipiens) bites on the skin was recorded. N, Ndiethyl-3 methyl benzamide was considered as positive control.

Results: The highest repelling property of the Sambucus ebulus L. belonged to the concentration of
250 mg/kg of leaf and fruit extraction. The highest repelling effect was 80% and 66.8% for leaf methalonic and hexanic extract, respectively. The highest repelling effect was 84% and 72% for fruit metalonic and hexanic extract, respectively (P<0.05).

Conclusion: The methalonic extract of Sambucus ebulus L. had higher repelling efficiency compared to the hexanic extract. The fruit extract also had better effect than the leaf extract.

Abstract

Background and Objective: The most important factor in the integrity of saphenous vein is the health degree of endothelium which guaranties the dilation of them after cronary bypass sergery. Kind of preservative soulution has a key role in endothelial protection. This study was done to evaluate Protective effect of perserved solutions of krebs contains verapamil, adrenaline and propranololol in comparision with Krebs and Heparinated blood on desqumation of the endothelial cell in Saphenous vein of Guinea pig.
Methods: This experimental study was done on 28 male Guinea pigs with 380±40g weight.for separating 3mm of saphenous vein rings and Measuring of rings nitric oxide released in preserving solutions: Krebs (K), Krebs plus propranolol (K+P), adrenaline (K+A) and verapamil (K+V) compaired with heparinized blood at 30, 45, 60 and 90 minutes after harvesting measured by micro plate Griess reaction. Rings also stained by H&E and examined by light microscopy to evaluate endothelial desqumation.
Results: Average concentration of nitric oxide (NO) in the Krebs plus Verapamil solution (K+V) Vs Heparinized Blood (HB), Krebs (K), Krebs plus Adrenaline (K+A) and Krebs plus Propanololol (K+P) revealed significant increase in NO release (P<0.05). The maximum NO measurement was 45 minuts after harvesting. Also histological study with H&E staining showed that endothelial layer was intact only in Krebs plus verapamil in compaired to control group, but in the other solutions the vascular intimal cells had suffered different degrees.
Conclusion: It seems that Krebs solutions containing verapamil has more efficiently to the proper functioning of the saphenous veins endothelium in animal modle.
