Abstract

Background & Objective: Surgery and /or radiotherapy are major local treatments for esophageal carcinoma. The results of radiotherapy alone have been unsatisfactory. The purpose of this study was to evaluate the treatment outcome of definitive chemoradiotherapy for patients with esophageal SCC.

 

Materials & Methods: This prospective histological descriptive study was done on 190 nonmetastatic esophageal SCC patients which received definitive chemoradiotherapy (55-64 Gy, Cisplatin 80-100 mg/m2 day 1-5FU 750-1000 mg/m2, 24h infusion days 1-4) with curative intent in cancer research center, Mashhad University of Medical Sciences between Jan 2000-Jan 2004. At least one course of chemotherapy was prescribed concurrently with radiotherapy.

 

Results: There were 99 male and 91female patients with the median age of 65 (range, 25-87). The median radiation dose prescribed was 60 Gy (range, 55-64) and median chemotherapy courses was 4 (range, 1-8). With a median follow-up time of 12 months (range, 3-60), one, two and three-year survival rates were 67.8%, 48.7% and 36.3% respectively and median survival was 22 months. Reduction of symptoms was shown in 84% of patients with dysphagia. 109 patients remained disease free with a median follow-up of 14 months (range, 3-60), among which 34 were followed more than 24 months. We found 81 cases (42.6%) with treatment failure during the follow-up (34 locoregional failure, 14 distant metastases, 33 distant metastases accompanied with locoreginal failure).

 

Conclusion: Although definitive chemoradiotherapy can result in long term disease free survival and even cure in a subset of patients with SCC of esophagus, locoregional failure and distant metastasis has remained the main causes of treatment failure.

Abstract

Background and Objective: Oxidative stress plays a major role in the structural and functional changes of the myocardium due to diabetes. This study was done to determine the effect of swimming training and arbutin supplement on cardiac antioxidant enzymes and oxidative stress in diabetic rats. Methods: In this experimental study, 42 male Wistar rats were randomly allocated into 6 groups including control, diabetes, Arbutin, diabetes+Arbutin, diabetes+exercise and diabetes+ exercise + Arbutin (combined). Diabetes induced using alloxan (90 mg/kg/bw, intraperitoneally). Arbutin (50 mg/kg/bw, ip) was administered for 5 days a week. The exercise consisted of swimming training at 5 min to 36 min per day, 5 days a week for 6 weeks. Renal Malondialdehyde, catalase level and superoxide dismutase (SOD) activity were evaluated in animals. Results: Diabetes significantly increased cardiac Malondialdehyde level and decreased cardiac SOD activity and catalase level (P<0.05). Six weeks of supplementation with Arbutin, swimming training and combined intervention significantly increased catalas level and superoxide dismutase activity compared to the diabetes group(P<0.05). Malondialdehyde level significantly reduced in combined and exercise groups in comparison with diabetic group (P<0.05). Conclusion: Regular training (swimming) and Antioxidant supplement (Arbutin) protect the cardiac tissue against diabetes-induced oxidative stress through their antioxidants capacity and the combination of the two interventions have synergic effect.

Abstract

Background and Objective: Genu varum is a lower limb deformity. This study was done to evaluate the effects of shoe aging on the general and directional contraction of ankle muscles during running in females with genu varum.
Methods: This case-control study was done on 30 female students (15 individuals with genu varum degree 2 and 15 healthy individuals) using new and old sneakers for 6 months in the sports biomechanics laboratory of the University of Mohaghegh Ardabili, Iran. Each subject was given a new pair of sneakers, and after the initial data collection, the subjects were asked to use them in their daily activities for at least an hour a day for 6 months. Electrical muscle activity was recorded using surface electromyography.
Results: There was no significant difference in the general and directional contractions if ankle muscles during the running phases.
Conclusion: Shoe aging has no effect on ankle joint co-contractions.
