Abstract

Background & Objective: No nutrient deficiency is as bad as Iodine deficiency which attributed to a broad spectrum of disorders like goiter, abortion, hypothyroidism, etc. Screening for hypothyroidism must be done before or just after pregnancy. The present study was conducted to evaluate serum TSH and urinary Iodine concentration in pregnant women in Gorgan several years after salt iodization in Iran was implemented. Materials & Methods: From December to March 2004, 97 pregnant women referred for prenatal care to the Deziani Hospital in Gorgan were selected. 2 cc of blood for serum TSH measurement and 2 cc of urine for Iodine concentration measurement were collected. Urine Iodine concentration was measured by Acid digestion and TSH concentration measured by ELISA method. Results: 97 women, 86 (89%) women had normally urinary Iodine, 11 women had Iodine deficiency, 9 women had mild Iodine deficiency (82%) and 2 women (18%) had moderate Iodine deficiency none of them had severe Iodine deficiency. Serum TSH was normal in 90 women and only in 7 women it was more than 4 and none of them had TSH less than 0.3. There were not any significant statistical correlation between age, weight, height, parity, goiter grading, salary, gestational age and urinary Iodine concentration. Conclusion: According to low prevalence of Iodine deficiency in this study Iodine programs were successful in Gorgan and the mean urinary Iodine is high so evaluating of Iodine consumption for preventing of Iodine overload is necessary. Because of complications of Iodine deficiency especially in the 1st trimester of pregnancy regular measurement of urinary Iodine must be done in reproductive ages before pregnancy or just after pregnancy.

Abstract

Background and Objective: Divorce as a life crisis can lead to emotional and behavioral problems. This study was done to determine the effectiveness of group based reality therapy based on Glasser choice theory on the general health and obsessive beliefs of divorced women.
Methods: In this quasi-experimental study 30 divorced women were non-randomly divided into the two interventions and control groups (15 participants in each group). Perior of the study the general health questionnaire (GHQ) (anxiety, social function, somatic symptoms and depression) and obsessive thoughts questionnaire (OBQ-44) (responsibility, perfectionisn and importance of thoughts) were completed by subjects in both groups. Counselling with Glasser approach was performed for intervention group through 8 group sessions once a week and lasted for 120 minutes. Control group received no intervention. At the end of the study, Post-test was performed from both groups.
Results: Group training based on reality therapy in intervention group's participants caused a significant reduction in all items of the two variables, general health and obsessive thoughts in compared to the control group (P<0.05).
Conclusion: Group based reality therapy based on Glasser choice theory improves general health of divorced women and reduces their obsessive beliefs.

Abstract

Background and Objective: Control of postoperative pain is one of the most important stages in the recovery of patients after surgery. This study was done to compare the effectiveness of combined Ondansetron and Apotel on the post-operative pain after surgery of upper limb fractures.
Methods: This double blind clinical trial study was done on 50 individual (41 male and 9 female) with upper limb fractures referring to 5 Azar hospital in Gorgan northern Iran during 2017. Patients were assigned (block randomization) into control and intervention groups. After the end of operation in the recovery phase, both groups received pain PCA (Patient Controlled Analgesia). In control group, the pain pump consisted of 2 grams of Apotel and in the intervention group; the pain pump consisted of 2 grams Apotel and 8 mg of ondansetron. Visual Analogue Score (VAS) was evaluated in both groups after surgery for 24 hours. Pain score of patients compared in the 2 groups during the 3 time intervals after surgery.
Results: 4 hours after upper limb fracture surgery, the mean pain was significantly decreased in the intervention group (3.20±0.707) compared to control group (3.64±0.569) (P<0.05). 12 hours after upper limb fracture surgery The Mean pain, in the intervention group (1.88±0.927) was significantly reduced in compare to control group (2.64±1.186) (P<0.05). 24 hours after upper limb fracture surgery, The Mean pain was significantly reduced in the intervention group (1.40±0.645) in compare to control group (2.08±0.997) (P<0.05).
Conclusion: This study showed that administration of compination of Apotele and Ondansetron in post-operative pain of upper limb fractures is effective than apotele alone.

Abstract

Background and Objective: Depression is the most common psychiatric diagnosis. It causes major health problems. Women are more likely to develop depression than men. This study was done to investigate the effect of group mindfulness-based cognitive therapy on reducing depression and improving the quality of life of female students.
Methods: This quasi-experimental study was performed on 26 female students. Students non-randomly were divided into control and intervention groups. Students in intervention group were received the two periods (8 sessions, 2 hours at week) of training in mindfulness-based cognitive therapy. All the participants filled out Beck depression inventory II (BDI-II) and World Health Organization Quality of Life – BREF (WHOQOL-BREF) in perior and after the study.
Results: In the intervention group the mean and standard deviation of pre-test and post-test scores of BDI-II was 20.23±6.98 and 13.38±4.97, respectively (P<0.05). In the intervention group, mean and standard deviation of pre-test and post-test scores of WHOQOL-BREF questionnaire was 66.77±13.39 and 86.84±10.60, respectively (P<0.05). Also, the mean scores of post-test of quality of life in all subgroups items  was significantly increased in compared to the pre-test in the intervention group (P<0.05).
Conclusion: Mindfulness-based cognitive therapy increases the quality of life and reduced depression in female students.

Abstract

Background and Objective: Most people who commit suicide suffer from psychiatric disorders, especially depression and substance-related disorders. However, major psychiatric disorders are not the sole risk factor for suicide, and other reasons, such as physical illness, may also be considered as an important factor for suicide ideation. This study was conducted to investigate the prevalence of suicidal ideation and its relationship with depression and the general health status of patients referred to addiction treatment centers in Iran.
Methods: This descriptive-analytical study was conducted on 689 patients referred to addiction treatment centers in Yazd, Khorasan Razavi, Mazandaran, Sistan and Baluchistan, Fars, East Azarbaijan, and Kermanshah Provinces who were undergoing maintenance treatment with methadone in 2017. In this multicenter study, the subjects were selected by two-stage sampling; in the first stage, seven provinces (clusters) were randomly selected, and in the second stage, the patients from the selected provinces were enrolled via convenience sampling. The instruments used in this study included the General Health Questionnaire, the Beck Depression Inventory, and the Beck Suicide Idea Scale.
Results: Of 689 patients, 218 (31.6%) had suicidal ideation. 21.91% (151 cases) of subjects had history of attempted to suicide. There was a significant relationship between suicidal ideation, depression, and general health status and its dimensions including physical symptoms, anxiety and sleep disorders, social functioning, and depression (P<0.05).
Conclusion: Suicidal ideation is present in one-third of the people who refer to addiction treatment centers in Iran. Depression and poor general health are major contributors to suicidal ideation.

Abstract

Background and Objective: Ventilator-associated pneumonia (VAP) is an important complication in intensive care unit (ICU) patients with endotracheal intubation. Finding potential early indicators of this condition can aid in reducing the disease burden. We aimed to investigate the relationship between VAP occurrence and serum levels of C-reactive protein (CRP), procalcitonin (PCT), and hemoglobin (Hgb) during ICU hospitalization of brain trauma patients.
Method: This descriptive-analytical study was carried out on brain trauma patients (99 male, 39 female), referring to the 5th Azar hospital in Gorgan, Iran, in the course of 2017. The patients were hospitalized in ICU with endotracheal intubation and mechanical ventilation. The diagnosis of VAP was confirmed by a 12-score rating based on chest radiographs, body temperature, white blood cell count, and sputum culture. After admission, serum CRP, PCT, and Hgb were documented daily for 6 days.
Results: According to the diagnostic criteria, VAP was confirmed in 41 patients (30%). Serum CRP and PCT levels on the 6th-day post-admission were significantly associated with VAP diagnosis, while Hgb levels did not differ significantly between VAP and non-VAP patients. Serum levels of CRP, PCT, and Hgb on the 6th day were not associated with age or sex.
Conclusion: Our results indicate that serum CRP and PCT levels are associated with the occurrence of VAP in ICU patients receiving mechanical ventilation. Therefore, these biomarkers could be utilized to warn physicians about the possibility of VAP, thereby reducing mortality rate and hospitalization length.
