اثر هشت هفته تمرین مقاومتی-هوازی بر سطوح فاکتور-1 مشتق از سلول استرومایی و فاکتور رشد مشتق از پلاکت در بیماران بای‌پس عروق کرونر: یک مطالعه کارآزمایی بالینی

زمینه و هدف: ﺗﻤﺮﻳﻨﺎت ورزشی در ﺑﻴﻤﺎران ﻗﻠﺒﻲ ﭘﺲ از ﻋﻤﻞ ﺑﺎی ﭘﺲ ﺷﺮﻳﺎن ﻛﺮوﻧﺮ (coronary artery bypass grafting: CABG) ﻣﻮﺟﺐ ﺑﻬﺒﻮد عملکرد قلبی عروقی از طریق افزایش چگالی مویرگی عضله قلبی ﻣﻲﺷﻮد. فاکتور رشد مشتق از پلاکت (PDGF) و فاکتور-1 مشتق از سلول استرومایی (SDF1) نقش مهمی در آنژیوژنز و تکثیر و تمایز سلولی دارند. این مطالعه به منظور تعیین اثر هشت هفته تمرین مقاومتی-هوازی بر سطوح فاکتور-1 مشتق از سلول استرومایی و فاکتور رشد مشتق از پلاکت در بیماران بای‌پس عروق کرونر انجام شد.

روش بررسی: در اﻳﻦ کارآزمایی بالینی 24 ﺑﻴﻤﺎر ﻗﻠﺒﻲ تحت عمل جراحی CABG ﺑﻪ روش ﻧﻤﻮﻧﻪﮔﻴﺮی در دﺳﺘﺮس و ﻫﺪﻓﻤﻨﺪ اﻧﺘﺨﺎب و ﺑﻪﻃﻮر ﺗﺼﺎدﻓﻲ در دو ﮔﺮوه ١2 ﻧﻔﺮی ﺗﻤﺮﻳﻦ ﺗﺮﻛﻴﺒﻲ و ﻛﻨﺘﺮل قرار گرفتند. ﮔﺮوه ﺗﻤﺮﻳﻦ ﺗﺮﻛﻴﺒﻲ ﺑﻪ ﻣﺪت ﻫﺸﺖ ﻫﻔﺘﻪ و ﻫﻔﺘﻪای ﺳﻪ ﺟﻠﺴﻪ ﭘﺮوﺗﻜﻞﻫﺎی ﻣﻨﺘﺨﺐ ﺗﻤﺮﻳﻨﻲ ﻫﻮازی و ﻣﻘﺎوﻣﺘﻲ را اﺟﺮا ﻛﺮدﻧﺪ. ﻗﺒﻞ و ﺑﻌﺪ از ﺑﺮﻧﺎﻣﻪ ﺗﻤﺮﻳﻨﻲ ارزﻳﺎﺑﻲﻫﺎی ﺗﺮﻛﻴﺐ ﺑﺪﻧﻲ و ﻧﻤﻮﻧﻪﮔﻴﺮی ﺧﻮﻧﻲ برای ﺳﻨﺠﺶ ﻣﺘﻐﻴﺮﻫﺎ اﻧﺠﺎم ﺷﺪ. به منظور اندازه‌گیری PDGF و SDF1 از روش آزمایشگاهی الایزا استفاده شد.

یافته‌ها: میزان PDGF در گروه مداخله در مقایسه با گروه کنترل افزایش آماری معنی‌داری داشت (P<0.05) اگرچه میزان SDF1a در گروه مداخله در مقایسه با گروه کنترل افزایش داشت؛ اما این افزایش از نظر آماری معنی‌دار نبود.

نتیجه‌گیری: اجرای ﻫﺸﺖ ﻫﻔﺘﻪ ﺗﻤﺮﻳﻦ ﺑﺎزﺗﻮاﻧﻲ ﺗﺮﻛﻴﺒﻲ ﺑﺎ تغییر در ﻣﻘﺎدﻳﺮ ﭘﻼﺳﻤﺎیی PDGF در ﺑﻴﻤﺎران CABG ﺑﺎﻋﺚ ارﺗﻘﺎء ﺳﺎزﮔﺎریﻫﺎی ﻓﻴﺰﻳﻮﻟﻮژﻳﻜﻲ ﻣﻄﻠﻮﺑﻲ در ﺳﻼﻣﺖ ﻗﻠﺐ و ﻋﺮوق میﺷﻮد.
