اثر تمرینات هوازی - مقاومتی بر سطوح پلاسمایی گیرنده فعال کننده تکثیر پروگسیزوم-1آلفا در بیماران کرونری پس از عمل جراحی پیوند عروق کرونر: یک مطالعه کارآزمایی بالینی

زمینه و هدف: بیماری عروق کرونر قلب از مهم‌ترین علل مرگ و میر در بیشتر کشورها و از جمله ایران است. انجام فعالیت‌های ورزشی بعد از عمل بای‌پس عروق کرونر (Coronary Artery Bypass Grafting: CABG) و سازگاری‌های ناشی از آن باعث افزایش چگالی مویرگی عضله اسکلتی و قلبی همراه با رشد مویرگ‌های جدید (Angiogenesis) و بهبود عملکرد قلبی - عروقی و توانایی عملکردی آنها ناشی از افزایش بیوژنز میتوکندریایی می‌شود. این مطالعه به منظور تعیین اثر تمرینات هوازی - مقاومتی بر سطوح پلاسمایی گیرنده فعال کننده تکثیر پروگسیزوم-1آلفا (PGC1α) در بیماران کرونری پس از عمل جراحی CABG انجام شد.

روش بررسی: این کارآزمایی بالینی روی 30 مرد در محدوده سنی 60-45 سال که تحت عمل جراحی CABG قرار گرفتند؛ انجام شد. بیماران به طور تصادفی در دو گروه 15 نفری مداخله و کنترل قرار گرفتند. گروه مداخله به مدت هشت هفته (3 جلسه در هفته) تمرینات مقاومتی - هوازی را انجام دادند و برای گروه کنترل مداخله‌ای صورت نگرفت. برای اندازه‌گیری لنفوسیت، نمونه گیری خونی ناشتا 48 ساعت قبل از شروع اولین جلسه تمرینی و همچنین 48 ساعت پس از آخرین جلسه تمرینی به عمل آمد. برای اندازه‌گیری PGC1α از روش آزمایشگاهی الایزا استفاده شد.

یافته‌ها: تمرینات هوازی - مقاومتی سبب افزایش سطح PGC1α در گروه مداخله نسبت به گروه کنترل گردید (P<0.05). تغییرات قبل و بعد بیماران در گروه مداخله افزایش معنی‌داری را در سطوح پلاسمایی PGC1α در مقایسه با گروه کنترل نشان داد (P<0.05).

نتیجه‌گیری: تمرینات ترکیبی (مقاومتی-هوازی) با افزایش سطوح سرمی PGC1α می‌تواند به عنوان فاکتور موثری در شروع فرایندهای رگ‌زایی و افزایش چگالی مویرگی نقش داشته باشد و به عنوان فاکتوری تاثیرگذار در افراد مختلف به‌ویژه بیماران قلبی - عروقی مورد استفاده قرار گیرد.
