تغییرات الگوی متیلاسیون ژن N-استیل ترانسفراز در بیماران بی پاسخ به خط اول درمان مایکوباکتریوم توبرکلوزیس

زمینه و هدف: ظهور مقاومت دارویی در مایکوباکتریوم توبرکلوزیس و به ویژه پیدایش سویه‌های مقاوم به دارو مشکلاتی را برای درمان و کنترل شیوع سل در سرتاسر جهان به وجود آورده است. این مطالعه به منظور ارزیابی تغییرات الگوی متیلاسیون ژن N-استیل ترانسفراز (N-acetyltransferase: NAT) در بیماران بی پاسخ به خط اول درمان مایکوباکتریوم توبرکلوزیس انجام شد.

روش بررسی: این مطالعه توصیفی - تحلیلی روی 200 بیمار مبتلا به سل انجام شد. بعد از تهیه نمونه خونی از روش نمک اشباع برای استخراج DNA استفاده گردید. سپس برای تعیین کمیت و کیفیت DNA از دو روش اسپکتروفوتومتری و الکتروفورز بر روی ژل اگارز استفاده شد. بررسی الگوی متیلاسیون ژن NAT1 به روش HRM انجام شد.

یافته‌ها: 34 بیمار به خط اول درمان مقاوم بودند. 18 بیمار دارای الگوی هایپرمتیلاسیون، 12 بیمار الگوی غیرمتیله و 4 بیمار الگوی هایپومتیله را نشان دادند. 166 بیمار به خط اول درمان مقاوم نبودند که از این میان 23 بیمار در گروه هایپرمتیله، 120 بیمار در گروه غیرمتیله و 23 نفر در ردیف هایپومتیله قرار داشتند. بین سطح متیلاسیون و مقاومت دارویی ارتباط آماری معنی‌داری وجود داشت (P<0.05).

نتیجه‌گیری: نتایج این مطالعه نشان داد که مقاوم بودن به خط اول درمان مایکوباکتریوم توبرکلوزیس با سطح متیلاسیون ژن N-استیل ترانسفراز مرتبط است.
