فراوانی آلل‌های s1، s2، m1 و m2 ژن vacA هلیکوباکترپیلوری جداشده از نمونه‌های بالینی

زمینه و هدف : عفونت هلیکوباکترپیلوری عامل بیماری‌زایی بسیاری از بیماری‌های معده از جمله زخم پپتیک، سرطان معده و لنفوم معده است. دلایل تنوع پیامدهای عفونت ناشی از هلیکوباکترپیلوری ممکن است با اختلاف در ژنوتیپ یا بیان عوامل بیماری‌زایی وابسته به باکتری و نیز عوامل محیطی و میزبان مرتبط باشند. این مطالعه به منظور تعیین فراوانی آلل‌های s1، s2، m1 و m2 ژن vacA هلیکوباکترپیلوری جداشده از نمونه‌های بالینی انجام شد.

روش بررسی : در این مطالعه مقطعی توصیفی - تحلیلی، نمونه‌گیری از 183 بیمار دچار اختلالات گوارشی مراجعه کننده به بخش آندوسکوپی بیمارستان امیرالمومنین (ع) کردکوی، استان گلستان در سال 1395 انجام شد. از هر بیمار دو نمونه بیوپسی از ناحیه آنتروم گرفته شد. نمونه اول با تست اوره‌آز و نمونه دوم با محلول فسفات بافر سالین ارزیابی شد. تست اوره‌آز 50 نمونه مثبت و استخراج DNA انجام شد. واکنش زنجیره‌ای پلی‌مراز (polymerase chain reaction) برای آلل‌های vacA انجام گردید. سپس ارتباط ترشح توکسین با علایم گوارشی نظیر درد شکم، درد معده، ریفلاکس، حالت تهوع، التهاب معده، خونریزی، زخم معده، سوزش، کم‌خونی و کاهش وزن ارزیابی شد.

یافته‌ها : در سویه‌های جداشده فراوانی آلل‌های s1، s2، m1 و m2 ژن vacA هلیکوباکترپیلوری به ترتیب 88 درصد، 6 درصد، 38 درصد و 70 درصد تعیین شدند. همچنین ژنوتیپ‌های s1/m1، s1/m2، s2/m1 و s2/m2 ژن vacA هلیکوباکترپیلوری به ترتیب 36 درصد، 58 درصد، صفر درصد و 6 درصد تعیین شدند. ترشح توکسین با علایم گوارشی از نظر آماری ارتباط معنی‌داری نداشت.

نتیجه‌گیری :. ژنوتیپ غالب بیماران دارای اختلالات گوارشی (58 درصد) s1/m2 تعیین شد و ژنوتیپ s2/m1 مشاهده نگردید.
