Abstract

Background & Objective: Nasopharynx could be affected by many tumors types such as carcinoma and lymphoma. These tumors present specific manifestations and symptoms. In rare cases these tumors can present unusual symptoms and without careful attention could be missed. In the present article we have described a rare case of nasopharyngeal tumor which a mass in cheeks.

Case presentation: A 72 year-old female who presented inflammation and a mass in left cheek, nasal obstruction in right nose was referred to CTS department and coronal view of paransal sinus was performed. Because sinuses were intact, patient was treated with antibiotic and antiallergic drugs. In spite of medical therapies, cheek mass growth continued and CTS was repeated in coronal and axial view. In these images, a mass in left Nasopharynx and a polypoid mass in right nasal cavity were found. Patient was undergone endoscopic surgery and biopsies were done from those sites .Pathologist reported a nasal polyp in right nose and an undifferentiated carcinoma of left Nasopharynx and cheek. Immunohistochemistry investigation for differentiated tumor from lymphoma was done and confirmed the lymphoma. Patient was refereed to oncologist for further treatment.

Conclusion: In management of patient we should consider unusual presentation of nasopharyngeal lymphoma with cheek mass which is an unusual presentation of this tumor.

Abstract

Background & Objectives: In spite of rapid development and arrival of newer medical imaging modalities such as computed tomography and magnetic resonance imaging, hysterosalpingography (HSG) has a great role among imaging techniques because of its enormous advantages. HSG is the standard screening test for the diagnosis of tubal infertility and can provide useful information about the uterine cavity. The aim of this study was to demonstrate the value of HSG in the evaluation of infertility and to diagnose the tubo-uterine causes of infertility in Tabriz.

Methods: In current study, 1260 patients were investigated for infertility by HSG, patients of both primary and secondary infertility were included except those who have history of acute pelvic infection and allergy to contrast media. This study was conducted at Day Medical Imaging Center in Tabriz-IRAN.

Results: HSG outlined normal uterus cavity, normal tube bilaterally with peritoneal spill in 1045 patients (82.94%) and 215 cases (17.06%) were detected with abnormalities. Among these patients, 78 cases (36.28%) detected with normal uterine shape and block of one tube, 32 (14.88%) patients shows both tubal blockages with normal uterus. There was bicurnate uterus in one patient (0.47%) with blockage of both tubes

Conclusion: The most common abnormal finding was normal uterus cavity with block of one or both tubes. Prevention of the pelvic inflammatory diseases, on time diagnosis and treatment of genital infectious, avoiding unnecessary pelvic surgeries, healthcare improvement and increasing the knowledge of society are the important decreasing factors in the women infertility.

Abstract

Background: Disorders of sex development (DSD) are a medical condition that affects the normal process of sexual  Various of the genes needed for gonad development have been identified by investigation of patients with disorders sex development (DSD).Phenotypes of patients with 46,XY DSD range from atonalism in female phenotype with complete external  genitalia to male phenotype with testicular regression. Individuals with 46,XYagonadiam show a wide range of clinical features and in some cases, there is not a clear diagnosis for this patients. We presented the clinical and molecular study a patient with 46,XY female without gonadal tissue.
Case presentation: A 27-year-old female was attended to our center because of primary amenorrhea. Ultrasonography did not show gonadal tissue including Mullerian structures, uterus, and Wolffian structures. Also, the patient had not streak gonad. We performed cytogenetic study and molecular analysis, including automated sequencing of the entire coding region of SRY gene, in the patient with agonadism. Our result showed 46,XY karyotype. Also, we noticed that molecular mutations in SRY are not identified as a cause of DSD female without a gonadal tissue. Laboratory examination showed that this case is a unique patient with 46,XYfemale agonadism that has no association with previously described.
Conclusions: The present case was a patient with 46, XYagonadism without hormonal or kidney defect and we did not detect mutation in SRY gene. To our knowledge, this case is a unique patient with 46,XYagonadism that has no association with previously described. So this case would be helpful for clinicians to assess 46,XY female patients without gonadal tissue.

Abstract

Background: Supernumerary teeth are known as the teeth in excess of the normal dentition. Multiple supernumerary teeth are usually observed as having syndromes. Conversely, multiple supernumerary teeth without any associated systemic conditions or syndromes are not common. 
Case presentation: This article documents an unusual case report, a male patient with non-syndromic 10 supernumerary teeth in both jaws, an incidental finding during routine radiographic examination. 
Conclusion: CBCT is the best diagnostic imaging method currently available for determining the relationships of teeth and the surrounding structures in cases with multiple impacted teeth or severe overlapping of impacted teeth.

Abstract

A Dentigerous Cyst (DC) is the most common developmental odontogenic cyst that mostly happens in patients between 10 to 30 years of age. It is usually completely asymptomatic and is discovered on a routine radiographic examination or to evaluate the reason for an erupted tooth. Radiographically, this cyst usually presents a well-defined unilocular radiolucency attached to CEJ of an unerupted tooth which can cause displacement of adjacent tooth or resorption.
This article reports a large infected DC in a 10-year-old male patient related to first and second premolar maxillary teeth, which did not erupt or even change their positions. DC was treated successfully by decompression. After 2 years follow up, there was no evidence of recurrence. In conclusion, marsupialization of cyst with no orthodontic intervention or tooth extraction is a conservative, efficient protocol that stimulates bone healing and promotes the eruption of unerupted teeth even if they are deeply crowded.

Abstract

This article reports a pelvic hydatid cyst in a 10-year-old boy who initially presented with right flank pain. Computed tomography (CT) revealed a cystic lesion containing internal densities, which was found to be a hydatid cyst between the bladder and rectum during surgery. The lesion was excised and sent to a pathology laboratory for further examination.

Abstract

Tuberculosis (TB) remains one of the world’s most persistent infectious diseases, with pulmonary involvement being its most frequent clinical presentation. However, extrapulmonary forms of TB, although relatively rare, continue to pose diagnostic and therapeutic challenges because of their atypical and nonspecific symptoms. This report presents the case of a 35-year-old male who was admitted with gradual, progressive facial swelling, without nasal obstruction, rhinorrhea, or respiratory complaints. Given the absence of conventional symptoms of TB, the patient underwent comprehensive diagnostic evaluations to rule out neoplastic, granulomatous, or infectious conditions. Investigations included fine needle aspiration (FNA), chest computed tomography (CT), incisional biopsy of the affected area, and Ziehl-Neelsen staining, which revealed the presence of acid-fast bacilli. Histological and microbiological examinations ultimately confirmed the diagnosis of primary sinonasal tuberculosis, an extremely rare manifestation of extrapulmonary TB. The patient was started on a standard anti-tubercular regimen and monitored closely for six months, during which substantial clinical improvement was observed. Facial swelling resolved, with no evidence of recurrence. This case illustrates the diagnostic complexity of atypical presentations of TB and emphasizes the importance of maintaining a high index of suspicion, especially in TB-endemic regions. Sinonasal tuberculosis can easily mimic neoplasms or other chronic inflammatory conditions, potentially leading to misdiagnosis. Timely recognition and proper management are crucial for improving patient outcomes. This report provides valuable insight into the limited literature on primary sinonasal TB without nasal obstruction and highlights the need for increased awareness among healthcare professionals when evaluating similar head and neck presentations.
